三联预康复策略对改善脑胶质瘤患者术后康复及生存情况的影响研究

Study on triple pre-rehabilitation strategy on improving postoperative rehabilitation and survival of glioma patients

目的观察三联预康复策略对改善脑胶质瘤患者术后康复及生存情况的影响。方法本研究为前瞻性研究,研究对象为南阳市第一人民医院2022年1月至2023年8月收治的108例脑胶质瘤患者。按照随机数字表将入组患者分别列为常规组和三联组,每组各54例。常规组实施常规康复管理,三联组实施三联预康复策略,比较两组患者的心理状态、营养状态、运动功能恢复情况,及生存质量。结果在不同管理模式下,三联组的焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于常规组;三联组的血清总蛋白(TP)、血清白蛋白(ALB)、血清转铁蛋白(TRF)均高于常规组;三联组的简化Fugl-Meyer运动功能评定量表(FMA)上肢、下肢评分均高于常规组;三联组的功能状态评分(KPS)、生活质量评分(QOL)分别为(82.33±10.45)分、(85.61±10.25)分,均高于常规组(77.66±10.28)分、(80.25±10.36)分;体力状态评分(ZPS)(2.32±0.28)分低于常规组(3.24±0.72)分,差异具有统计学意义(P<0.05)。结论三联预康复策略能减轻脑胶质瘤患者术后应激程度,对改善患者心理状态、促进认知功能及肢体运动功能恢复,并改善短期生存质量均有积极意义。

Objective To observe the impact of the triple pre-rehabilitation strategy on improving postoperative recovery and survival of glioma patients.Methods This was a prospective study involving 108 glioma patients admitted to Nanyang First People's Hospital from January 2022 to August 2023.The enrolled patients were randomly assigned into conventional group(54 cases)and triple group(54 cases)using computer random grouping method.Patients in the conventional group were intervened by regular rehabilitation management,while those in the triple group were intervened by triple pre-rehabilitation strategy.Psychological status,nutritional status,recovery of motor function,and quality of life were compared between the two groups.Results Under different management modes,the self-rating anxiety scale(SAS)and the self-rating depression sscale(SDS)scores of the triple group were significantly lower than those of the conventional group(P<0.05).Total protein(TP),serum albumin(ALB)and serum transferrin(TRF)were significantly higher than those of the conventional group(P<0.05).The simplified Fugl-Meyer assessment(FMA)of motor recovery of upper and lower limb scores were significantly higher than those of the conventional group(P<0.05).The karnofsky performance scale(KPS,[82.33±10.45]points VS[77.66±10.28]points)and the quality of life(QOL,[85.61±10.25]points VS[80.25±10.36]points)scores were significantly higher than those of the conventional group(P<0.05).The Zubrod ECOG WHO performance status(ZPS)scores([2.32±0.28]points VS[3.24±0.72]points)were significantly lower than those of the conventional group(P<0.05).Conclusion The triple pre-rehabilitation strategy can alleviate the postoperative stress level of glioma patients,and improve their psychological state,cognitive and limb motor function recovery,and short-term quality of life.
