血清survivin、ABCA1与急性心肌梗死单支冠脉闭塞患者冠脉侧支循环不良的关系分析

Analysis of the relationship between serum survivin,ABCA1 and poor coronary collateral circulation in patients with single coronary artery occlusion in acute myocardial infarction

摘要 目的探讨血清生存素(survivin)、三磷酸腺苷结合盒转运体A1(ABCA1)与急性心肌梗死(AMI)单支冠脉闭塞患者冠脉侧支循环(CCC)不良的关系。方法选取2021年5月至2023年5月在邯郸市第一医院收治的155例AMI单支冠脉闭塞患者作为研究对象,根据其CCC情况分为CCC不良组(n=80)和CCC良好组(n=75)。比较两组血清survivin、ABCA1水平,采用单因素和多因素Logistic回归模型分析CCC不良的影响因素,以及采用受试者工作特征曲线分析血清survivin、ABCA1预测CCC不良的效能。结果与CCC良好组比较,CCC不良组血清survivin、ABCA1显著降低,差异有统计学意义(P<0.05)。与CCC良好组比较,CCC不良组Gensini评分、肌酸激酶同工酶(CK-MB)、Killip心功能分级≥Ⅱ级占比、高血压史占比显著升高,差异有统计学意义(P<0.05)。多因素Logistic回归模型显示,高Gensini评分、高CK-MB、Killip心功能分级≥Ⅱ级、有高血压史是CCC不良的危险因素(P<0.05),高survivin、高ABCA1为其保护因素(P<0.05);两项指标联合预测CCC不良的曲线下面积、灵敏度、特异度依次为0.949、90.70%、92.52%,显著优于单项检测。结论血清survivin、ABCA1与AMI单支冠脉闭塞患者CCC不良密切相关,血清survivin、ABCA1水平越低者CCC不良风险越大,两者联合检测对AMI单支冠脉闭塞患者CCC不良有较高的预测价值。 Objective To investigate the relationship between serum survivin,adenosine triphosphate binding cassette transporter A1(ABCA1)and poor coronary collateral circulation(CCC)in patients with single coronary artery occlusion in acute myocardial infarction(AMI).Methods A total of 155 patients with single coronary artery occlusion in AMI admitted to Handan Fist Hospital from May 2021 to May 2023 were selected as the study objects,and were divided into poor CCC group(n=80)and good CCC group(n=75)according to their CCC status.Serum survivin and ABCA1 levels were compared between the two groups,univariate and multivariate Logistic regression models were used to analyze the influencing factors of poor CCC,and the efficacy of serum survivin and ABCA1 in predicting poor CCC was analyzed by receiver operating characteristic curve.Results Compared with good CCC group,serum survivin and ABCA1 in poor CCC group were significantly decreased,and the difference was statistically significant(P<0.05).Compared with good CCC group,Gensini score,creatinine kinase isoenzyme-MB(CK-MB),Killip heart function grade≥Ⅱand history of hypertension in poor CCC group were significantly increased,and the differences were statistically significant(P<0.05).Multivariate Logistic regression model showed that high Gensini score,high CK-MB,Killip heart function grade≥Ⅱand history of hypertension were risk factors for poor CCC(P<0.05),while high survivin and high ABCA1 were protective factors(P<0.05).The area under the curve,sensitivity and specificity of the two indexes combined to predict poor CCC were 0.949,90.70%and 92.52%,which were significantly better than the single detection.Conclusion Serum survivin and ABCA1 are closely relate to poor CCC in patients with single coronary artery occlusion in AMI,the lower the serum levels of survivin and ABCA1,the greater the risk of poor CCC,the combine detection of the two has a high predictive value for poor CCC in patients with single coronary artery occlusion in AMI.
