探析从“久病入络”论治卵巢巧克力囊肿纤维化

The treatment of ovarian chocolate cysts fibrosis from“chronic diseases transforming to collaterals”

卵巢巧克力囊肿是子宫内膜浸润内陷到卵巢皮质形成的慢性囊肿,纤维化是卵巢巧克力囊肿的重要病理特征,微血管病变是卵巢巧克力囊肿纤维化的重要病理基础。中医认为,卵巢巧克力囊肿为久病痼疾。卵巢巧克力囊肿的病理机制涉及血管新生及微血管障碍,其治疗多从“瘀”而论。“络病”的病理基础亦可总结为“瘀”,包括各种病因引起机体微循环、微血管及微环境的调节失常。由此可见,纤维化与中医“络病”具有病理相通性,此种久病痼疾更符合“久病入络”的病理特点。络以通为要,虫药搜剔经络在临床取得显著疗效。笔者从“久病入络”角度探析卵巢巧克力囊肿纤维化的中医辨治,以期为临床治疗提供新思路。

Ovarian chocolate cysts are chronic cysts formed by endometrial infiltration and invagination into the ovarian cortex.Fibrosis is an important pathological feature of ovarian chocolate cysts,and microangiopathy is the pathological basis of fibrosis in ovarian chocolate cysts.According to TCM,ovarian chocolate cysts are chronic diseases.The pathological mechanism of ovarian chocolate cysts involves neovascularization and microvascular disorders,and its treatment is mostly based on“blood stasis”.The pathological basis of the“collateral disease”can also be summarized as“blood stasis”,including the malfunction of microcirculation,microvessel and microenvironment caused by various causes.Thus,it can be seen that fibrosis and the“collateral disease”in Chinese medicine have the pathological similarity,and this kind of chronic disease is more in line with the pathological characteristics of“chronic diseases transforming to collaterals”.The treatment is based on“dredging”,insect medicine treatment of meridians in the clinic has achieved significant efficacy.Therefore,the author explores the Chinese medicine treatment of ovarian chocolate cyst fibrosis from the perspective of“chronic diseases transforming to collaterals”,in order to provide a new idea for clinical treatment.
