基于“少火生气,壮火食气”探讨肺癌病机及治疗

Exploring the pathogenesis and treatment of lung cancer based on“mild fire generating qi and sthenic fire consuming qi”

肺癌的发病率和死亡率居于我国恶性肿瘤之首,其预后不佳。基于“少火生气,壮火食气”理论,将古代医家记载与现代医学研究结合,明确“少火”“壮火”内涵,气与火两者关系,从“少火生气,壮火食气”切入到肺癌的发病机制,指出“少火衰”是其病机的本质,“少火”不行,正气渐虚,“壮火盛”是其病机的关键,痰热瘀毒内生,虚实兼夹,气火失调,“食气”为表象,贯穿病程始终。从病机探讨中得出治疗思路,“培少火,清壮火,防传变”是论治肺癌的核心,包含三方面:益气温阳,培土生金以助少火生气;泻火解毒,凉血养阴以平壮火食气;消痰散结,祛瘀生新,谨防变证。

Lung cancer has the highest incidence rate and mortality among malignant tumors in China,and its prognosis is poor.Based on the theory of“mild fire generating qi and sthenic fire consuming qi”,combining ancient medical records with modern medical research,clarifying the connotations of“mild fire”and“sthenic fire”,as well as the relationship between qi and fire,and starting from“mild fire generating qi and sthenic fire consuming qi”to explore the pathogenesis of lung cancer.It is pointed out that“mild fire”is the essence of its pathogenesis,“sthenic fire”is the key to its pathogenesis.Phlegm,heat,stasis,and toxins are generated in the body,resulting in a combination of deficiency and excess,as well as an imbalance of qi and fire.“Consuming qi”is the surface manifestation that runs through the entire course of the disease.From the exploration of the pathogenesis,the treatment approach is derived.“Cultivating mild fire,clearing sthenic fire,and preventing transmission”is the core of the treatment of lung cancer,which includes three aspects.Nourishing qi and warming yang,cultivating soil and generating gold to help mild fire consuming qi.Purge fire and detoxify,cool blood and nourish yin to eliminate sthenic fire consuming qi.Dispelling phlegm and nodules,removing blood stasis and generating new vitality,and guarding against changes in symptoms.
