多孔腹腔镜手术治疗卵巢癌的疗效及对外周血T淋巴细胞、术后并发症的影响

Effect of Porous Laparoscopic Surgery on Ovarian Cancer and Its Influence on Peripheral Blood T Lymphocytes and Postoperative Complications

目的探究多孔腹腔镜手术治疗Ⅰ~Ⅲ期卵巢癌患者的疗效及对外周血T淋巴细胞、术后并发症的影响。方法选取Ⅰ~Ⅲ期卵巢癌患者86例作为研究对象,根据治疗方案进行分组,其中,行开腹手术的46例患者为对照组,行多孔腹腔镜手术的40例患者为观察组。以围手术期指标、生存质量评分、外周血T淋巴细胞水平以及术后并发症发生情况作为观察指标,评估两种治疗方案。结果2组患者的各项指标在治疗前无显著性差异(P>0.05)。2组手术时间无显著性差异(P>0.05);治疗后,与对照组相比,观察组淋巴结清扫数、生存质量评分、CD3^(+)、CD4^(+)以及CD4^(+)/CD8^(+)水平均明显更高(P<0.05),术中出血量、首次排气时间、胃肠功能恢复时间、CD8^(+)水平、术后并发症总发生率均明显更低(P<0.05)。结论多孔腹腔镜手术治疗卵巢癌患者,可有效改善患者围手术期指标和外周血T淋巴细胞水平,减少术后并发症,明显提高患者生存质量,具有较高的临床应用价值。

Objective To investigate the efficacy of multiport laparoscopic surgery for the treatment of patients with stageⅠtoⅢovarian cancer and its effect on peripheral blood T lymphocytes and postoperative complications.Methods 86 patients with stageⅠ~Ⅲovarian cancer who underwent surgical treatment were selected as study subjects and grouped according to the treatment protocols,of which 46 patients who underwent open surgery were the control group and 40 patients who underwent multi-hole laparoscopic surgery were the observation group.Perioperative indicators,survival quality score,peripheral blood T-lymphocyte level and the occurrence of postoperative complications were used as observational indicators to evaluate the 2 treatment plans.Results There was no significant difference between the indicators of the 2 groups before treatment(P>0.05).There was no significant difference in the operation time between the 2 groups(P>0.05);After treatment,compared with the control group,the number of lymph node clearance,quality of survival score,CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)levels in the observation group were significantly higher(P<0.05);intraoperative hemorrhage,time to first gas evacuation,time to recovery of gastrointestinal function,CD8^(+)level,and the total incidence of postoperative complications were significantly lower(P<0.05).Conclusion Porous laparoscopic surgery for ovarian cancer patients can effectively improve the perioperative indicators and the level of peripheral blood T lymphocytes,reduce postoperative complications,significantly improve the quality of life of patients,and has high clinical application value.
