雄激素受体的差异分级表达与乳腺癌患者的临床病理特征的关联性分析

摘要 目的分析不同表达的雄激素受体(AR)与乳腺癌患者的临床特征及免疫组化的关联性。方法选取郑州大学第一附属医院2018-01—2019-06收治的232例乳腺癌患者,根据AR表达量的差异进行分组,其中AR表达量<10%为低表达组、10%~50%为中表达组、大于50%为高表达组。统计分析三组乳腺癌患者的不同AR表达量与临床病理特征的关系,以及与其他乳腺癌免疫组化指标(ER、PR、Her-2、ki-67以及P53)之间的关联性。结果232例乳腺癌患者中,AR低表达69例(29.7%)、AR中表达55例(23.7%)、AR高表达108例(46.6%)。较AR低表达和中表达患者,AR高表达患者具有组织学分级低、ER阳性率高,PR高表达,以及ki-67低表达的特征(P<0.05)。AR的差异表达与年龄、月经状态、肿瘤直径、有无淋巴结转移、病理类型以及有无脉管癌栓无相关性(P>0.05)。结论乳腺癌患者中,AR阳性高表达与较低的组织学分级、较高的ER阳性率,PR高表达,以及较低的Ki-67增殖指数等指示患者有较好预后的临床病理指标存在高度关联性。以上的研究结果提示,就乳腺癌患者而言,AR很可能成为一种新型的特异性治疗靶点和高特异性及敏感性的预后指标。 Objective To analyze the differential expression of androgen receptor(AR)in breast cancer patients and its correlation with clinicopathological features and immunohistochemistry.Methods A total of 232 breast cancer patients admitted to the First Affiliated Hospital of Zhengzhou University from 2018 to 2019 were selected as the study objects,and divided into groups according to the differences in AR expression levels,in which AR expression levels less than 10%were considered as low expression group,10%~50%as medium expression group,and more than 50%as high expression group.The relationship between different expression levels of AR and clinicopathological features of breast cancer patients was statistically analyzed,and the correlation between different expression levels of AR and other immunohistochemical indicators of breast cancer(ER,PR,Her-2,ki-67 and P53)was also analyzed.Results Among 232 breast cancer patients,69 cases(29.7%)had low AR expression,55 cases(23.7%)had medium AR expression,and 108 cases(46.6%)had high AR expression.Compared with patients with low and medium expression of AR,patients with high expression of AR had lower histological grade,higher positive rate of ER,higher expression of PR,and lower expression of Ki-67(P<0.05).There was no correlation between AR expression and age,menstrual status,tumor diameter,lymph node metastasis,pathological type and vascular cancer thrombus(P>0.05).Conclusion High AR positive expression in breast cancer patients is associated with low histological grade,high ER positive rate,high PR expression,low Ki-67 proliferation index and good prognostic clinicopathological indicators,suggesting that AR may be a therapeutic target and prognostic indicator for breast cancer.
