程序化疼痛管理对老年股骨颈骨折患者苏醒期躁动的影响

Effect of programmed pain management on restlessness during recovery in elderly patients with femoral neck fracture

目的探讨程序化疼痛管理对老年股骨颈骨折患者苏醒期躁动的影响。方法选取2020年5月至2021年5月吉安市中心人民医院收治的60例股骨颈骨折患者作为研究对象,按照随机数字表法分为观察组(n=30)和对照组(n=30)。两组患者均接受髋关节置换术治疗,对照组采用常规护理,观察组采用程序化疼痛管理。比较两组患者术后苏醒时间、拔管时间、躁动评分[Riker镇静躁动评分量表(SAS)]、康复指标[髋关节评分量表(HHS)、改良Barthel指数(MBI)]、疼痛程度[视觉模拟评分法(VAS)]。结果两组患者术后苏醒时间、拔管时间比较,差异无统计学意义(P>0.05);观察组SAS、VAS评分低于对照组,观察组HHS、MBI评分高于对照组,差异有统计学意义(P<0.05)。结论程序化疼痛管理能够有效改善老年股骨颈骨折患者苏醒期躁动情况,提高髋关节功能,改善日常生活,降低疼痛感。

Objective To explore the effect of programmed pain management on restlessness during recovery in elderly patients with femoral neck fracture.Methods A total of 60 patients with femoral neck fracture admitted to Ji'an Central People's Hospital from May 2020 to May 2021 were selected as the study objects,they were divided into the observation group(n=30)and the control group(n=30)according to random number table method.Both groups received hip replacement,the control group received conventional care,and the observation group received programmed pain management.Postoperative recovery time,extubation time,agitation score(Riker sedation-agitation scale[SAS]),rehabilitation index(Harris hip score[HHS]),(modified Barthel index[MBI])and pain degree(visual analogue scale[VAS])were compared between the two groups.Results There were no significant differences in postoperative recovery time and extubation time between the two groups(P>0.05).The scores of SAS and VAS in the observation group were lower than those of the control group,and the scores of HHS and MBI in the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Programmed pain management can effectively improve restlessness,hip function,daily life and pain perception in elderly patients with femoral neck fracture.
