头针为主治疗中风后失语临床及机制研究进展

Research Progress on Clinical and Mechanism of Scalp Acupuncture in the Treatment of Post-Stroke Aphasia

通过检索近10年头针为主治疗中风后失语(PSA)临床研究文献,对头针应用概况、头针操作方法、头针临床疗效、头针机制研究四个方面进行归纳分析,总结头针为主治疗中风后失语的临床及机制研究现状。结果表明,头针应用以焦氏头针最为广泛;头针治疗PSA疗效显著;头针结合语言康复治疗是目前的研究热点;《汉语失语症检查法》量表是目前最常用的结局指标;头针机制主要针对脑电活动、脑部代谢产物、相关血液流变学指标的影响进行研究。但存在头针操作不规范、真实世界研究较少、疗效评价不统一且缺乏中医特色评价及相关核心结局指标集、作用机制研究偏少等问题。应当进一步规范化、标准化头针操作,开展更多真实世界研究,构建中医治疗PSA核心结局指标集并采用循证医学方法评价头针为主治疗PSA的疗效、开展更多机制研究。

By searching for clinical research literature on the use of scalp acupuncture as the main treatment for post-stroke aphasia(PSA)in the past 10 years,we conducted a summary and analysis of the clinical application,operation methods,clinical efficacy,and mechanism research of scalp acupuncture as the main treatment for PSA,and summarized the current status of clinical and mechanism research on scalp acupuncture as the main treatment for PSA.The results showed that Jiao’s scalp acupuncture was the most widely used,the therapeutic effect of scalp acupuncture on PSA is significant,and the combination of scalp acupuncture and language rehabilitation were the current research hotspots.The Chinese Aphasia Examination Method scale is the most commonly used outcome indicator.The mechanism research of scalp acupuncture mainly focuses on the effects of scalp acupuncture on EEG activity,brain metabolites,and related hemorheology indicators.However,there are problems such as non-standard scalp acupuncture operation,limited real-world research,inconsistent efficacy evaluation,lack of evaluation of traditional Chinese medicine characteristics and related core outcome indicators,and insufficient research on the mechanism of action.We should further standardize the operation of scalp acupuncture,conduct more real-world research,construct a core outcome indicator set for traditional Chinese medicine treatment of PSA,and use evidence-based medicine methods to evaluate the efficacy of scalp acupuncture as the main treatment for PSA,and conduct more mechanism research.
