血液病患者PICC导管堵塞风险预测指标体系的构建

摘要 目的构建血液病患者PICC导管堵塞风险预测指标体系, 为尽早识别和预防导管堵塞提供依据。方法检索PICC导管堵塞的国内外文献、指南和行业标准, 用思维导图提取风险因素, 邀请25名多学科专家进行两轮Delphi专家函询, 确定血液病患者PICC导管堵塞风险预测指标体系。结果两轮函询专家权威系数是0.92, 积极系数分别为88%和84%, 肯德尔协调系数为0.24(χ^(2)=529.32, P<0.05)。经两轮函询, 制定了包含4个一级指标、10个二级指标、28个三级指标的血液病患者PICC导管堵塞预测风险指标体系。结论本研究制定的血液病患者PICC导管堵塞预测风险指标体系科学可靠, 对于尽早识别和预防血液病患者PICC导管堵塞具有指导意义。 ObjectiveTo develop an indicator system for evaluating PICC obstruction risk in patients with blood diseases,so as to provide a basis for early identification and prevention of catheter occlusion.MethodsDomestic and foreign literature,guidlines and industry standard on PICC catheter occlusion were searched,and risk factors were extracted with mind mapping.A total of 25 multidisciplinary experts were invited to conduct two rounds of Delphi expert correspondence consultation to determine the risk prediction index system for PICC catheter occlusion in patients with blood diseases.ResultsThe authority coefficient of experts in the dtwo rounds of consultation was 0.92,the positive coefficients were 88%and 84%,respetively.The Kendall coordination coefficient was 0.24(χ^(2)=529.32,P<0.05).After two rounds of expert consultation,the indicator system for evaluating PICC obstruction risk in patients with blood diseases was developed,and it consisted of 4 first-level indicators,10 second-level indicators and 28 third-level indicators.ConclusionsThe indicator system for evaluating PICC obstruction risk in patients with blood diseases approved by experts is scientific and reliable,and it provides guidance for early identification and prevention of PICC catheter occlusion in patients with blood diseases.
