中药硬膏贴联合生物反馈治疗脾肾阳虚型出口梗阻型便秘的临床疗效观察

Clinical efficacy of Chinese medicine hard paste combined with biofeedback in treatment of outlet obstruction constipation of spleen and kidney yang deficiency-type

目的探讨中药硬膏贴与生物反馈相结合治疗脾肾阳虚型出口梗阻型便秘的效果。方法选择河南中医药大学第一附属医院肛肠病诊疗中心2022年11月—2023年11月期间收治的60例出口梗阻型便秘患者,通过计算机随机数字表法将其划分为治疗组和对照组,每组30例。治疗组使用中药硬膏贴,并在微波炉中加热至45℃,然后将其贴在关元穴上,每次持续8 h,每天进行1次,并配合生物反馈进行治疗。对照组采用生物反馈治疗,每日一次。两组1周为1个疗程,共2个疗程,每个疗程间隔1天。结果治疗组的治愈例数达到了17例,其中11例显效,2例好转,0例无效,有效率达到了93.33%;对照组的治愈例数为9例,其中15例显效,6例好转,0例无效,有效率达到80%。两组治疗后,便秘症状(周排便次数、粪便性状、排便困难程度)、心理相关指标(焦虑自评量表、抑郁自评量表评分)和生活质量指标(生活质量量表)均优于治疗前,但治疗组优于对照组(P<0.01),差异有统计学意义。结论中药硬膏贴联合生物反馈,比单纯使用生物反馈,更能够改善脾肾阳虚型出口梗阻型便秘患者的便秘症状、心理状况和生活质量。

Objective To explore the effect of combining Chinese medicine hard paste and biofeedback in the treatment of outlet obstruction constipation of spleen and kidney yang deficiency type.Methods Sixty patients with outlet obstruction type constipation were selected and divided into two groups,and each group has 30 people.In the treatment group,Chinese herbal medicine hard paste was used and heated to 45℃ in microwave oven,and then it was applied to Guanyuan acupoints for 8 hours each time,which was carried out once a day with biofeedback for treatment.The control group was treated with biofeedback once a day.The two groups took 1 week as 1 course of treatment,and there were 2 courses of treatment in total,with an interval of 1 day between each course of treatment.Results The number of cured cases in the treatment group reached 17 cases,of which 11 cases showed effect,2 cases improved,0 case was invalid,and the effective rate of these cases reached 93.33%;while the number of cured cases in the control group was 9 cases,of which 15 cases showed effect,6 cases improved,0 case was invalid,and the effective rate of these cases reached 80%.After treatment,the constipation symptoms(number of weekly defecation,faecal character,degree of defecation difficulty),psychological related indexes(anxiety self-assessment scale,depression self-assessment scale scores)and quality of life indexes(quality of life scale)of the two groups were better than those before the treatment,however,the Chinese medicine group was better,and the difference was statistically significant.Conclusion The combination of Chinese medicine hard paste and biofeedback is more capable of improving the constipation symptoms,psychological condition and quality of life of patients with outlet obstruction type constipation with spleen and kidney yang deficiency than the use of biofeedback alone.
