急性缺血性脑卒中患者血清miR-132、sFasL及IL-1β水平与血管性痴呆的相关性

摘要 目的探讨急性缺血性脑卒中(AIS)患者血清微小RNA-132(miR-132)、可溶性细胞凋亡因子配体(sFasL)及白细胞介素-1β(IL-1β)水平与血管性痴呆(VD)的关系。方法选取2021年1月至2022年12月佳木斯市中心医院东院区神经内科收治的103例AIS患者,根据是否发生VD分为VD组(36例)和非VD组(67例)。统计两组患者基础资料,对比两组血清miR-132、sFasL、IL-1β水平。分析血清miR-132、sFasL及IL-1β对AIS患者发生VD的预测价值。结果VD组年龄≥60岁、多发性梗死、基底核梗死患者比例高于非VD组,脑梗死面积>非VD组(P<0.05)。VD组患者血清miR-132水平低于非VD组,而血清sFasL、IL-1β水平高于非VD组(P<0.05)。VD患者血清miR-132水平与MoCA评分呈正相关,血清sFasL、IL-1β水平与MoCA评分呈显著负相关(P<0.05)。Logistic回归分析显示,脑梗死面积、年龄、基底核区梗死、血清miR-132、sFasL及IL-1β水平是AIS患者发生VD的独立影响因素(P<0.05)。血清miR-132、sFasL、IL-1β及联合检测预测AIS患者发生VD的曲线下面积(AUC)为0.840、0.797、0.824、0.926,联合预测的AUC及敏感性显著高于单项检测(P<0.05)。结论血清miR-132、sFasL、IL-1β水平与AIS患者VD的发生有关,miR-132、sFasL联合IL-1β检测有助于早期预测VD发生风险。 Objective To investigate the relationship between serum levels of microRNA-132(miR-132),soluble Fas ligand(sFasL),interleukin-1β(IL-1β)and vascular dementia(VD)in patients with acute ischemic stroke(AIS).Methods 103 AIS patients admitted to department of neurology in Jiamusi central hospital east campus from January 2021 to December 2022 were divided into VD group(36 cases)and non-VD group(67 cases)according to whether VD occurred.The basic data of two groups were analyzed.The serum miR-132,sFasL and IL-1β levels were compared between the two groups.The predictive value of miR-132,sFasL and IL-1β on VD in AIS patients was analyzed.Results The proportion of patients aged≥60 years,multiple infarction and basal ganglia infarction in VD group were higher than those in non-VD group,and the cerebral infarction area was greater than that in non-VD group(P<0.05).The serum miR-132 level in VD group was lower than that in non-VD group,and the serum sFasL and IL-1β levels were higher than those in non-VD group(P<0.05).Serum miR-132 levels were positively correlated with MoCA scores,and serum sFasL and IL-1β levels were negatively correlated with MoCA scores(P<0.05).Logistic regression analysis showed that cerebral infarction size,age,basal ganglia infarction,serum sFasL and IL-1β levels were independent risk factors for VD in AIS patients(P<0.05).The area under the curve(AUC)of serum miR-132,sFasL,IL-1β and combined detection to predict VD in AIS patients were 0.840,0.797,0.824 and 0.926,and the AUC and sensitivity of combined detection were significantly higher than those of single detection(P<0.05).Conclusion The serum miR-132,sFasL and IL-1β levels are related to the incidence of VD in AIS patients,miR-132 and sFasL combined with IL-1β detection is helpful for early prediction of VD risk.
