复方多黏菌素B软膏辅助治疗对特应性皮炎患儿湿疹面积、EASI评分及不良反应发生率的影响

Effects of Compound Polymyxin B Ointment Adjuvant Therapy on EASI Score and the Incidence of Adverse Reactions in Children with Atopic Dermatitis

目的探究复方多黏菌素B软膏辅助治疗对特应性皮炎(AD)患儿湿疹面积及严重程度指数评分(EASI)及不良反应发生率的影响。方法本研究采用随机数字表法将江西省儿童医院自2020年1月至2023年9月收治的80例AD患儿分为观察组和对照组,各40例。对照组接受地奈德乳膏治疗,在此基础上,观察组接受复方多黏菌素B软膏辅助治疗,比较两组临床疗效、EASI评分、嗜酸性粒细胞水平、炎性因子水平、不良反应以及复发率。结果观察组临床总有效率高于对照组(95.00%vs 80.00%,P<0.05)。治疗2周后,观察组湿疹面积评分、湿疹严重程度评分、EASI总评分均低于对照组(P<0.05);观察组嗜酸性粒细胞、白细胞介素-4、干扰素-γ、肿瘤坏死因子-α水平低于对照组(P<0.05)。两组不良反应发生率比较(12.50%vs 5.00%,P>0.05)差异无统计学意义,对照组复发率高于观察组(18.75%vs 2.63%,P<0.05)。结论复方多黏菌素B软膏辅助治疗对AD患儿临床效果显著,有效降低湿疹面积,缓解湿疹严重程度,降低EASI总评分,改善嗜酸性粒细胞水平,促进康复,且安全性良好,复发率较低。

Objective To investigate the effects of compound polymyxin B ointment adjuvant therapy on eczema area and severity index(EASI)score and the incidence of adverse reactions in children with atopic dermatitis(AD).Methods Using the random number table method,80 children with AD who were admitted to the Jiangxi Provincial Children's Hospital from January 2020 to September 2023 were divided into an observation group and a control group,with 40 cases in each group.The control group was treated with desonide cream,on this basis,the observation group was given adjuvant therapy with compound polymyxin B ointment.Clinical efficacy,EASI score,eosinophil level,inflammatory factor levels,adverse reactions,and recurrence rate were compared between the two groups.Results The total clinical response rate in the observation group was higher than that in the control group(95.00%vs 80.00%,P<0.05).After 2 weeks of treatment,eczema area score,eczema severity score and total EASI score of the observation group were lower than those of the control group(P<0.05);the levels of eosinophils,interleukin-4,interferon-γand tumor necrosis factor-αin the observation group were significantly lower than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(12.50%vs 5.00%,P>0.05).The recurrence rate in the control group was higher than that in the observation group(18.75%vs 2.63%,P<0.05).Conclusion Compound polymyxin B ointment adjuvant therapy is effective for children with AD.It can effectively reduce the area and severity of eczema,lower the total EASI score,improve eosinophil level,and promote rehabilitation,with good safety and a low recurrence rate.
