新生儿低血糖与脑易损性的相关性研究

摘要 目的探究新生儿低血糖与脑易损性的相关性。方法选择2017年6月—2023年2月本院收治的90例低血糖症新生儿进行回顾性分析,根据新生儿是否发生脑损伤分为脑损伤组和脑未损伤组,其中脑损伤组31例,脑未损伤组59例。检测患儿血糖水平、首次出现低血糖时间、低血糖持续时间,以及脑损伤血清学指标[神经元特异性烯醇化酶(NSE)、髓鞘碱性蛋白(MBP)、神经胶质纤维酸性蛋白(GFAP)和S100蛋白(S100)],并分析低血糖与前述血清指标间相关性。结果两组首次诊断低血糖时间,差异无统计学意义(P>0.05)。脑未损伤组血糖水平明显高于脑损伤组,且低血糖持续时间明显比脑损伤组短,差异有统计学意义(P<0.05)。脑损伤组新生儿静脉血NSE、MBP、GFAP和S100水平明显比脑未损伤组高,差异有统计学意义(P<0.05)。Pearson相关性分析显示,新生儿低血糖与血清NSE、MBP、GFAP、S100呈正相关,差异有统计学意义(P<0.05)。结论新生儿低血糖严重程度与脑易损性呈正相关。 Objective To explore correlation between neonatal hypoglycemia and brain vulnerability.Methods The paper reviewed and analyzed 90 newborns with hypoglycemia admitted to our hospital from June 2017 to February 2023,and divided them into brain injury group and non brain injury group based on whether they had brain injury.Among them,there were 31 cases in brain injury group and 59 cases in non brain injury group.Blood glucose levels,time of first occurrence of hypoglycemia,duration of hypoglycemia,and serum indicators of brain nerve injury specific enolase(NSE),myelin basic protein(MBP),glial fibrillary acidic protein(GFAP),and S100 protein(S100)were detected,correlation was analyzed between hypoglycemia and the aforementioned serum indicators.Results There was no significant difference in initial diagnosis time of hypoglycemia between two groups(P>0.05).Blood glucose level in non brain injury group was significantly higher thanbrain injury group,duration of hypoglycemia was significantly shorter than brain injury group(P<0.05).Levels of NSE,MBP,GFAP,and S100 in venous blood of newborns in brain injury group were significantly higher than non brain injury group(P<0.05).Pearson correlation analysis showed,neonatal hypoglycemia was positively correlated to serum NSE,MBP,GFAP,and S100(P<0.05).Conclusion Severity of neonatal hypoglycemia is positively correlated with brain vulnerability.
