摘要
目的基于奥马哈系统(Omaha system,OS)构建重症肌无力(myasthenia gravis,MG)患者的延续性护理模式并进行临床应用评价。方法成立护理方案制定小组,回顾性收集MG患者2021年1月—12月的临床病历资料132例,并基于OS问题系统制定MG患者的延续性护理模式。制定延续性护理模式后,自2022年1—12月招募入院的MG患者,并依据随机数字表将117例MG患者分为对照组(常规护理模式,58例)和OS护理模式组(基于OS问题系统制定的延续性护理模式,59例)。采用简短身体活动量表、肌无力症状评估量表、肌无力疾病活动量表、心理量表、护理满意度评分,评价OS护理模式组在MG患者中的临床应用价值。结果基于OS问题系统成功构建10条主要护理措施;OS护理模式组中,MG患者身体活动功能评分及护理满意度评分均高于对照组,组间差异有统计学意义(P<0.05);疾病严重程度得分、临床症状及肌力评价得分、焦虑抑郁得分、知觉应激评分均显著低于对照组,组间差异有统计学意义(P<0.05)。结论基于OS构建的延续性护理模式能够有效改善MG患者的临床治疗效果,值得推广使用。
Objective To construct a continous nursing model for patients with myast henia gravis(MG)based on Omaha system(OS)and evaluate its clinical application.Methods The paper reviewed clinical case data of MG patients from January 2021 to December 2021,and established a nursing plan development team,developed continous nursing model for MG patients based on OS problem system.The paper divided MG patients in the hospital from January 2022 to December 2022 into control group and OS nursing model group with random number table,and evaluated clinical application value of OS nursing model for MG patients with Short Physical Activity Scale,Myasthenia Symptom Assessment Scale,Myasthenia Disease Activity Scale,Psychological Scale,and Nursing Satisfaction Scores.Results There are ten primary nursing interventions successfully established based on the OS problem system.Physical activity function scores and nursing satisfaction degree of MG patients in OS nursing model group were higher than control group,difference was statistically significant(P<0.05).Disease severity scores,clinical symptom and muscle strength evaluation scores,anxiety and depression scores,and perceived stress scores were significantly lower than control group,difference was statistically significant(P<0.05).Conclusion Continuous nursing model based on OS can improve clinical treatment effect of MG patients effectively,which is worthy of promotion and application.
