Ⅳ期非小细胞肺癌患者营养风险及营养相关血液学指标分析

Analysis of Nutritional Risk and Nutrition-related Hematological Indexes in Patients with StageⅣNon-small Cell Lung Carcer

目的:探讨Ⅳ期非小细胞肺癌患者营养风险发生情况及营养相关血液学指标的变化,并通过指标分析为营养支持提供一定的依据。方法:采用回顾性研究方法,收集安徽医科大学第二附属医院肿瘤科2020年1月—2023年1月收治的92例首次确诊Ⅳ期非小细胞肺癌患者的临床资料,包括年龄、性别、体重指数(BMI)、营养风险筛查2002(NRS 2002)评分、血液学指标。根据年龄将患者分为老年组(66~89岁)和中青年组(18~65岁),并分析两组营养不足发生情况和血液学指标异常情况。结果:92例Ⅳ期非小细胞肺癌患者营养风险发生率为22.83%,66~89岁患者营养风险发生率明显高于18~65岁患者,差异有统计学意义(χ^(2)=8.573,P=0.003)。Ⅳ期非小细胞肺癌患者营养不足发生率为13.04%(12/92),老年组营养不足发生率(21.57%)显著高于中青年组(2.44%),差异有统计学意义(P=0.007)。Ⅳ期非小细胞肺癌患者血液指标异常率较高,超过25%的患者存在白蛋白(ALB)、血红蛋白(Hb)、白细胞(WBC)计数、淋巴细胞绝对值(LYM)。老年组白蛋白(ALB)、钾(K)、镁(Mg)异常率均明显高于中青年组,WBC异常率低于中青年组,差异均有统计学意义(P<0.05)。结论:Ⅳ期非小细胞肺癌患者存在较高的营养风险,其营养相关血液学指标显著异常。

Objective:To investigate the occurrence of nutritional risk and the changes of nutritionrelated hematological indexes in patients with stage Ⅳ non-small cell lung cancer,and to provide a basis for nutritional support through indexes analysis.Method:Clinical data of 92 patients with stage Ⅳ non-small cell lung cancer diagnosed for the first time in Department of Oncology,the Second Hospital of Anhui Medical University from January 2020 to January 2023 were collected retrospectively,including age,gender,body mass index(BMI),utritional risk screening 2002(NRS 2002)score and hematological indexes.The patients were divided into old group(aged 66 to 89)and young and middle-aged group(aged 18 to 65)according to age,and the occurrence of malnutrition and abnormal hematological indexes in the two groups were analyzed.Result:The incidence of nutritional risk in 92 patients with stage Ⅳ non-small cell lung cancer was 22.83%.The incidence of nutritional risk in patients aged 66 to 89 was significantly higher than that in patients aged 18 to 65,the difference was statistically significant(χ^(2)=8.573,P=0.003).The incidence of undernutrition in patients with stage Ⅳ non-small cell lung cancer was 13.04%(12/92).The incidence of undernutrition in the old group(21.57%)was significantly higher than that in the young and middle-aged group(2.44%),the difference was statistically significant(P=0.007).Patients with stage Ⅳ non-small cell lung cancer had a high abnormal rate of blood indexes,and more than 25% of patients had abnormal albumin(ALB),hemoglobin(Hb),white blood cell(WBC)count,absolute lymphocyte value(LYM).The abnormal rates of albumin(ALB),potassium(K)and magnesium(Mg)in the old group were significantly higher than those in the young and middle-aged group,the abnormal rate of WBC was lower than that of the young and middle-aged group,the differences were statistically significant(P<0.05).Conclusion:Patients with stage Ⅳ non-small cell lung cancer have higher nutritional risk,and their nutritionrelated hematological indexes are significantly abnormal.
