独活寄生汤联合常规西药治疗膝骨关节炎患者的效果

Effects of Duhuo Jisheng decoction combined with conventional Western medicine in treatment of patients with knee osteoarthritis

目的:观察独活寄生汤联合常规西药治疗膝骨关节炎(KOA)患者的效果。方法:选取2021年12月至2023年12月该院收治的80例KOA患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各40例。对照组采用常规西药(塞来昔布+硫酸氨基葡萄糖)治疗,观察组在对照组基础上联合独活寄生汤治疗。比较两组临床疗效,治疗前后中医证候积分、关节功能[骨关节炎指数(WOMAC)]评分、炎性因子[白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、C反应蛋白(CRP)]水平,以及不良反应发生率。结果:观察组治疗总有效率为97.50%(39/40),高于对照组的75.00%(30/40),差异有统计学意义(P<0.05);治疗后,观察组主症、次症等中医证候积分和WOMAC评分均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组IL-6、CRP、IL-1β水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:独活寄生汤联合常规西药治疗KOA患者可提高治疗总有效率,降低中医证候积分、关节功能评分和炎性因子水平,效果优于单纯常规西药治疗。

Objective:To observe effects of Duhuo Jisheng decoction combined with conventional Western medicine in treatment of patients with knee osteoarthritis(KOA).Methods:A prospective study was conducted on 80 patients with KOA admitted to the hospital from December 2021 to December 2023.According to the random number table method,they were divided into control group and observation group,40 cases in each group.The control group was treated with conventional Western medicine(Celecoxib+Glucosamine sulfate),while the observation group was treated with Duhuo Jisheng decoction on the basis of that of the control group.The clinical efficacy,the TCM syndrome scores,the joint function[Western Ontario and McMaster University of orthopedic index(WOMAC)]score,the inflammatory factors[interleukin-6(IL-6),interleukin-1β(IL-1β),C-reactive protein(CRP)]levels before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 97.50%(39/40),which was higher than 75.00%(30/40)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of TCM syndromes such as main symptoms and secondary symptoms and the WOMAC score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of IL-6,CRP and IL-1βin the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Duhuo Jisheng decoction combined with conventional Western medicine in the treatment of the KOA patients can improve the total effective rate of treatment and reduce the TCM syndrome scores,the joint function scores and the inflammatory factor levels.Moreover,it is superior to simple conventional Western medicine treatment.
