达州地区10425例女性人乳头瘤病毒感染状况及基因型分析

Analysis of human papilloma virus infection status and genotypes in 10 425 women in Dazhou area

目的分析达州地区女性人乳头瘤病毒(HPV)感染状况及基因型分布特征,为宫颈癌的防治及疫苗接种提供参考依据。方法收集自2021年11月至2023年10月在达州市中西医结合医院采用荧光PCR方法检测HPV基因型的10425例女性宫颈脱落上皮细胞样本,对所得的样本数据进行统计分析。结果10425例中,HPV感染2504例,感染率为24.02%;感染率排前3的年龄段为>18~20岁(35.29%)、>60岁(33.23%)、>50~60岁(29.51%),不同年龄段HPV感染率比较,差异有统计学意义(P<0.01)。感染类型以高危型感染为主,高危型感染亚型前4位为HPV52、58、16、53型,低危型感染亚型前3位为HPV81、44、42型;>20岁高危型感染以HPV52、58、16型为主,>18~20岁以HPV52、68、16型为主。除>18~20岁外,其他年龄段女性均以单一感染为主,>18~20岁多重感染占比最高(17.65%),其次是>60岁(15.13%)。结论达州地区女性HPV感染以单一高危型感染为主,HPV52、58、16、53型是感染较多的高危型,>18~20岁和>60岁的女性应做好防治措施。

Objective To analyze the infection status and genotypes distribution of human papilloma virus(HPV)in women in Dazhou,and provide reference for prevention and treatment of cervical cancer and vaccination.Methods Samples of cervical exfoliated epithelial cells were collected from 10425 women with HPV genotypes detected by fluorescent PCR in Dazhou Integrated Traditional Chinese and Western Medicine Hospital from November 2021 to October 2023,and the resulting sample data were statistically analyzed.Results Among the 10425 cases,2504 cases were infected with HPV,and the infection rate was 24.02%.The top three age bracket of infection rate were>18-20 years old(35.29%),>60 years old(33.23%),>50-60 years old(29.51%).There was a significant difference in HPV infection rate among different age bracket(P<0.01).The main infection types were high-risk infection,the top 4 highrisk infection types were HPV52,58,16,53,and the top 3 low-risk infection types were HPV81,44,and 42.HPV52,58,and 16 were the main types in>20 years old,while HPV52,68,and 16 were the main types in>18-20 years old.In addition to>18-20 years old,the majority of women in other age bracket were single infection,and the proportion of multiple infection in>18-20 years old was the highest(17.65%),followed by>60 years old(15.13%).Conclusion The main type of HPV infection in women in Dazhou area is single high-risk type.HPV52,58,16,and 53 are the high-risk types with more infection.Women>18-20 years old and>60 yea rs old should take prevention and control measures.
