常规疗法、吡非尼酮联合乙酰半胱氨酸治疗老年特发性肺纤维化的临床效果分析

Analysis on Clinical Effect of Conventional Therapy,Pirfenidone Combined with Acetylcysteine in Treatment of Elderly Idiopathic Pulmonary Fibrosis

目的:分析常规疗法、吡非尼酮联合乙酰半胱氨酸治疗老年特发性肺纤维化(IPF)的临床效果。方法:选取2022年3月—2023年8月黄石南健康复护理院收治的老年IPF患者70例作为研究对象,随机分为对照组和观察组,各35例。两组均给予常规疗法,对照组给予吡非尼酮治疗,观察组在对照组基础上给予乙酰半胱氨酸治疗。比较两组治疗效果。结果:治疗后,观察组动脉血氧分压(PaO_(2))、氧合指数、pH高于对照组,动脉血二氧化碳分压、透明质酸、Ⅲ型胶原、Ⅳ型胶原、白细胞介素-1β、可溶性髓系细胞触发受体-1水平低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率高于对照组,差异有统计学意义(P=0.046)。结论:常规疗法、吡非尼酮联合乙酰半胱氨酸治疗老年IBF的临床效果显著,可改善患者动脉血气指标、肺间质纤维化指标,降低炎性因子水平。

Objective:To analyze the clinical effect of conventional therapy,pirfenidone and acetylcysteine in treatment of elderly idiopathic pulmonary fibrosis(IPF).Methods:A total of 70 elderly patients with IPF who were admitted to Huangshi Nanjian Rehabilitation Nursing Hospital from March 2022 to August 2023 were selected as the study subjects.They were randomly divided into control group and observation group,with 35 cases in each group.Both groups received conventional therapy,the control group was treated with pirfenidone,and the observation group was treated with acetylcysteine on the basis of the control group.The treatment effects were compared between the two groups.Results:After treatment,the arterial partial pressure of oxygen(PaO_(2)),oxygenation index and pH in the observation group were higher than those in the control group,while the levels of arterial partial pressure of carbon dioxide,hyaluronic acid,typeⅢcollagen,typeⅣcollagen,interleukin-1βand soluble triggering receptor expressed on myeloid cells-1(sTREM-1)in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.046).Conclusion:Conventional therapy,pirfenidone combined with acetylcysteine has significant clinical effects in the treatment of elderly IBF,and can improve arterial blood gas indexes and pulmonary interstitial fibrosis indexes,and reduce the levels of inflammatory factors.
