摘要
目的:探讨减压植骨术联合胫骨横向骨搬移术治疗早期股骨头坏死的临床效果。方法:选取2020年9月—2023年9月右江民族医学院附属西南医院收治的早期股骨头坏死患者60例作为研究对象,按照随机数字表法分为对照组和观察组,各30例。对照组采用经大粗隆单一孔道减压植骨术治疗,观察组在对照组基础上采用胫骨横向骨搬移术治疗。比较两组髋关节功能、疼痛评分。结果:术前、术后1 d,两组髋关节评分量表(Harris)评分比较,差异无统计学意义(P>0.05);术后90 d,两组Harris评分高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。术前、术后1 d,两组视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05);术后90 d,两组VAS评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:减压植骨术联合胫骨横向骨搬移术治疗早期股骨头坏死的临床效果显著,能够改善患者髋关节功能,缓解术后疼痛。
Objective:To explore the clinical effect of decompression and bone grafting combined with tibial transverse transport in the treatment of early femoral head necrosis.Methods:A total of 60 patients with early femoral head necrosis admitted to the Affiliated Southwest Hospital of Youjiang Medical University for Nationalities from September 2020 to September 2023 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 30 cases in each group.The control group was treated with single-core decompression and bone grafting through the greater trochanter and the observation group was treated with tibial transverse transport on the basis of the control group.The hip joint function and pain scores of the two groups were compared.Results:There was no significant difference in the scores of hip joint rating scale(Harris)between the two groups before surgery and on 1 d after surgery(P>0.05).On 90 d after operation,the Harris scores in the two groups were higher than those before surgery,the scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the scores of visual analogue scale(VAS)between the two groups before surgery and on 1 d after surgery(P>0.05).On 90 d after surgery,the VAS scores in the two groups were lower than those before surgery,the scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Decompression and bone grafting combined with tibial transverse transport has a significant clinical effect in the treatment of early femoral head necrosis,and can improve hip joint function and relieve postoperative pain.
