探讨脑梗死静脉溶栓后早期使用替罗非班对进展性脑梗死的影响

The effect of early use of tirofiban on progressive cerebral infarction after intravenous thrombolysis inpatients with cerebral infarction

目的探讨脑梗死静脉溶栓后早期使用替罗非班是否可降低进展性脑梗死的发生率。方法60例脑梗死患者,随机分为观察组及对照组,每组30例。观察组静脉溶栓后2~12 h内使用替罗非班治疗,持续24 h后桥接口服抗血小板聚集药物;对照组静脉溶栓1 d后予以抗血小板聚集药物治疗。对比两组进展性脑梗死发生率、临床疗效。结果观察组进展性脑梗死发生率3.33%低于对照组的20.00%,差异有统计学意义(P<0.05)。观察组患者临床总有效率96.67%显著高于对照组的76.67%,差异有统计学意义(P<0.05)。结论脑梗死静脉溶栓后早期使用替罗非班可巩固静脉溶栓的效果,降低进展性脑梗死的发生率。

Objective To explore whether early use of tirofiban after intravenous thrombolysis in patients with cerebral infarction can reduce the incidence of progressive stroke.Methods 60 patients with cerebral infarction from were randomly divided into an observation group and a control group,with 30 cases in each group.The observation group was treated with tirofiban within 2-12 h after intravenous thrombolysis,and was then treated with oral antiplatelet aggregation drugs for 24 h.The control group was treated with antiplatelet aggregation drugs after 1 d of intravenous thrombolysis.The incidence of progressive cerebral infarction and clinical efficacy were compared between the two groups.Results The incidence of progressive cerebral infarction in the observation group was 3.33%,which was lower than 20.00%in the control group,and the difference was statistically significant(P<0.05).The total clinical effective rate of the observation group was 96.67%,which was significantly higher than 76.67%of the control group,and the difference was statistically significant(P<0.05).Conclusion Early use of tirofiban after intravenous thrombolysis for cerebral infarction can consolidate the effect of intravenous thrombolysis and reduce the incidence of progressive cerebral infarction.
