Pharmaceutical monitoring of toxoplasma treatment associated with cytomegalovirus pneumonia in heart transplant patients

临床药师全程参与1例心脏移植患者弓形虫伴巨细胞病毒肺炎临床治疗方案制订,协同临床医生调整用药方案,同时为患者提供个体化用药监护,包括药物相关不良反应监测及处理,保证患者的用药安全,取得良好的临床结局。临床药师实施个体化用药监护有助于减少药物相关不良事件的发生,改善症状,提高临床治疗结果,同时也可促进临床合理用药,保障患者用药安全。

Throughout the treatment process of one patient of toxoplasmosis and cytomegalovirus(CMV) pneumonia,clinical pharmacist participated in formulating clinical treatment protocol,cooperated with clinicians in adjusting medications and provided individualized medication monitoring and managed drug-related adverse reactions to ensure the safety of drug use for better clinical outcomes.Implementing individualized medication monitoring by clinical pharmacist helped to lower the occurrence of drugrelated adverse events,improve symptoms and optimize outcomes.Meanwhile,it could promote rational clinical drug use and ensure the safety of patients.
