摘要
目的:探讨胸部多层螺旋CT在波伦综合征(PS)中的诊断价值。方法:回顾性收集2013年12月—2023年12月山东大学齐鲁医院(青岛)收治的13例波伦综合征患者的胸部多层螺旋CT影像及临床资料,分析波伦综合征的临床与影像特征。结果:患者均单侧发病,其中3例合并上肢畸形,1例迷走右锁骨下动脉,13例患者均有胸大肌中下束或上中下束缺如,12例胸小肌缺如或短小,1例前锯肌部分缺如,3例肋骨及软骨畸形,6例胸骨畸形,5例乳腺腺体减少,6例乳头大小或位置改变。胸部多层螺旋CT可显示胸肌缺损、肋骨及肋软骨畸形、乳腺减少和乳头减小及可能合并的心脏大血管畸形。多平面重建及容积再现技术可多角度、立体显示异常改变。结论:胸部多层螺旋CT是诊断波伦综合征的有效影像学方法。
Objective To investigate the diagnostic value of chest multi-slice spiral CT in Poland syndrome.Methods Retrospective collection of chest multi-slice spiral CT images and clinical data of 13 patients with Poland syndrome admitted to Qilu Hospital of Shandong University(Qingdao)from December 2013 to December 2023,analyzing the clinical and imaging characteristics of Poland syndrome.Results Unilateral onset was observed in all patients,with 9 cases on the right side and 4 cases on the left side.Among them,upper limb deformity was present in 3 cases,vagal right subclavian artery in 1 case,absence of middle and lower pectoral bundle or upper and lower pectoral bundle in 13 cases,absence or shortness of pectoralis minor in 12 cases,partial absence of serratus anterior in 1 case,rib and cartilage deformity in 3 cases,sternum deformity in 6 cases,reduced breast glands in 5 cases,and changes in nipple size or position were noted among 6 cases.Chest multi-slice spiral CT can effectively visualize defects of pectoral muscles,rib and rib cartilage deformities as well as reduced breast glands and nipples.Additionally it can potentially identify combined cardiovascular malformations.Multi-plane reconstruction along with volume rendering techniques enable comprehensive evaluation from multiple angles providing a three-dimensional view.Conclusion Chest multi-slice spiral CT is an effective imaging method for the diagnosis of Poland syndrome.
