基于气相色谱-质谱联用技术检测阻塞性睡眠呼吸暂停儿童血浆代谢变化的研究

摘要 通过气相色谱-质谱(gas chromatography-mass spectrometry,GC-MS)技术研究阻塞性睡眠呼吸暂停(obstructive sleep apnea,OSA)儿童血浆代谢物的变化,以寻找儿童OSA的候选代谢标志物及诊断方法。选取在我院确诊为OSA的儿童5例及健康儿童7例,进行多导睡眠监测(polysomnography,PSG)检查,次日清晨抽取静脉血3 mL后利用GC-MS对差异代谢物进行鉴定分析,并富集相应的代谢通路。结果发现,与健康对照组相比,OSA患儿共筛选出105种潜在的差异代谢物,表达上调有65个,表达下调有40个;差异代谢物富集在29条代谢信号通路,其中12条具有显著差异,包括丙氨酸、天冬氨酸和谷氨酸代谢、柠檬酸盐循环、色氨酸代谢、组氨酸代谢、乙醛酸和二羧酸代谢、苯丙氨酸、酪氨酸和色氨酸的生物合成等。OSA患儿血浆中氨基酸、脂肪酸和碳水化合物等代谢物含量的明显变化可能是OSA儿童发病的潜在机制。 Plasma metabolite changes in children with obstructive sleep apnea(OSA)were studied by gas chromatography-mass spectrometry(GC-MS)to find candidate metabolic markers and diagnostic methods for OSA in children.Five children diagnosed with OSA in our hospital and seven healthy children were selected.After polysomnography(PSG)examination,3 mL of venous blood was withdrawn for GC-MS metabolomics analysis in the early morning of the next day to identify and analyze the differential metabolites and enrich the corresponding metabolic pathways.The results revealed that a total of 105 potential differential metabolites were screened in children with OSA compared with healthy controls,with 65 up-regulated and 40 down-regulated expressions.The differential metabolites were enriched in 29 metabolic signaling pathways,of which 12 were significant different,including alanine,aspartate and glutamate metabolism,citrate cycle,tryptophan metabolism,histidine metabolism,glycolate and dicarboxylic acid metabolism,and phenylalanine,tyrosine and tryptophan biosynthesis,etc.Plasma levels of amino acids,fatty acids,and carbohydrates were significantly altered in children with OSA,and changes in these metabolites may be a potential mechanism for the pathogenesis of children with OSA.
