内镜下腺样体、扁桃体切除术治疗小儿阻塞性睡眠呼吸暂停低通气综合征的效果分析

Clinical efficacy of endoscopic adenoidectomy combined with tonsillectomy in the treatment of obstructive sleep apnea hypopnea syndrome in children

目的探究内镜下腺样体、扁桃体切除术治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的效果。方法队列研究。抽取2022年4月至2023年6月榆林市第一医院收治的OSAHS患儿110例,根据随机数字表法分为对照组与研究组,每组55例。对照组给予持续气道正压通气治疗,研究组给予内镜下行腺样体、扁桃体切除术治疗。对比两组患儿治疗前后多导睡眠仪监测指标和治疗效果。结果治疗前,两组患儿多导睡眠仪监测指标比较差异未见统计学意义(P>0.05);治疗后,两组患儿呼吸暂停低通气指数、最低动脉血氧饱和度(SaO_(2))<92%的时间占比均下降,最低SaO_(2)数值上升,且研究组变化幅度大于对照组(P<0.05)。研究组治疗总有效率(94.55%,52/55)高于对照组(81.82%,45/55),P<0.05。结论内镜下腺样体、扁桃体切除术可有效改善OSAHS患儿临床通气指标与症状,治疗效果较好。

Objective To investigate the therapeutic effect of endoscopic adenoidectomy combined with tonsillectomy on obstructive sleep apnea hypopnea syndrome(OSAHS)in children.Methods A total of 110 children with OSAHS admitted to Yulin First Hospital from April 2022 to June 2023 were selected for the cohort study.And they were divided int the control group and the study group using a random number table method,with 55 cases in each group.The control group was treated by continuous positive airway pressure ventilation,while the study group was treated by endoscopic adenoidectomy combined with tonsillectomy.The preoperative and postoperative indexes of polysomnography and therapeutic effect were compared between the two groups.Results Before treatment,there was no significant difference in the indexes of polysomnography between the two groups(P>0.05);after treatment,the apnea-hypopnea index and the proportion of the time of the lowest arterial oxygen saturation(SaO_(2))below 92%decreased in both groups,while the lowest SaO_(2) increased in both groups,moreover,the changes in the study group were more obvious compared with the control group(P<0.05).The total effective rate of the study group(94.55%,52/55)was higher than that of the control group(81.82%,45/55),P<0.05.Conclusions Endoscopic adenoidectomy combined with tonsillectomy can effectively improve the clinical ventilatory parameters and symptoms in children with OSAHS,of which the therapeutic effect is better.
