关节镜微创手术联合针灸康复治疗肩袖损伤的临床研究

Effect of arthroscopic minimally invasive surgery combined with acupuncture and moxibustion rehabilitation on rotator cuff injury

目的探讨关节镜微创手术联合针灸康复治疗肩袖损伤的临床效果。方法队列研究。抽取2022年1月至2023年1月延安市中医医院收治的80例肩袖损伤患者,按照随机数字表法分为对照组和观察组,每组40例。对照组实施关节镜微创手术,观察组采用关节镜微创手术联合针灸康复治疗,两组均持续干预6周。比较两组手术疗效及干预期间并发症发生情况;比较两组干预前、干预6周后肩关节功能[美国肩肘外科协会(ASES)评分、视觉模拟评分法(VAS)评分]。结果观察组治疗优良率(95.00%,38/40)高于对照组(80.00%,32/40),(P<0.05)。干预6周后,两组ASES评分高于干预前,VAS评分低于干预前(P<0.05);干预6周后,观察组ASES评分高于对照组,VAS评分低于对照组(P<0.05)。观察组并发症发生率(5.00%,2/40)与对照组(10.00%,4/40)比较差异未见统计学意义(P>0.05)。结论关节镜微创手术联合针灸康复治疗肩袖损伤的效果显著,可有效改善患者肩部功能,利于患者术后恢复。

Objective To investigate the treatment effect of arthroscopic minimally invasive surgery combined with acupuncture and moxibustion rehabilitation on rotator cuff injury.Methods A total of 80 patients with rotator cuff injury admitted to Yan’an City Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were selected for the cohort study.And they were divided into a control group and an observation group by random number table method,with 40 cases in each group.The control group was treated by arthroscopic minimally invasive surgery,and the observation group was treated by arthroscopic minimally invasive surgery combined with acupuncture and moxibustion rehabilitation treatment.Both groups underwent intervention for 6 weeks continuously.The treatment efficacy and incidence of complications during intervention were compared between the two groups.The shoulder joint function assessed by American Association of Shoulder and Elbow Surgeons(ASES)score and visual analogue scale(VAS)score in the two groups were compared.Results The excellent and good rate of the observation group(95.00%,38/40)was higher than that of the control group(80.00%,32/40),P<0.05.After 6 weeks of intervention,ASES score in both groups increased,and VAS score decreased compared with the scores before intervention(P<0.05).The observation group had higher ASES score and lower VAS score compared with the control group after 6 weeks of intervention(P<0.05).There was no statistically significant difference in the incidence of complications between the observation group(5.00%,2/40)and the control group(10.00%,4/40),P>0.05.Conclusions Arthroscopic minimally invasive surgery combined with acupuncture and moxibustion rehabilitation has a significant therapeutic effect on rotator cuff injury.It can effectively improve the shoulder function,and is conductive to postoperative rehabilitation.
