长期血液透析留置导管患者血栓发生的影响因素

Influencing factors of thrombosis in patients with long-term catheterization for hemodialysis

目的探讨长期血液透析留置导管患者血栓发生的影响因素。方法回顾性抽取2021年6月至2023年4月郑州大学第一附属医院收治的108例接受长期血液透析留置导管的慢性肾衰竭患者为研究对象,记录所有患者置管6个月内血栓发生情况。统计所有患者血液透析前凝血酶原(PT)、活化部分凝血活酶时间(APTT)、C反应蛋白(CRP)等实验室指标及糖尿病病史、年龄等基线资料,通过Logistic回归分析长期血液透析留置导管患者血栓发生的影响因素。结果置管6个月内,108例长期血液透析留置导管患者中发生血栓21例(19.44%),未发生血栓87例(80.56%)。发生组年龄大于未发生组,PT、APTT、CRP水平高于未发生组,糖尿病者占比高于未发生组(P<0.05)。经Logistic回归分析结果显示,年龄大、有糖尿病病史、PT水平高、APTT水平高、CRP水平高是长期血液透析留置导管患者血栓发生的影响因素(P<0.05)。结论年龄大、有糖尿病病史、PT水平高、APTT水平高、CRP水平高是长期血液透析留置导管患者血栓发生的影响因素。

Objective To investigate the influencing factors of thrombosis in patients with long-term catheterization for hemodialysis.Methods A total of 108 patients with chronic renal failure who received long-term catheterization for hemodialysis in the First Affiliated Hospital of Zhengzhou University from June 2021 to April 2023 were selected as the study subjects.The incidence of thrombosis within 6 months of catheterization was recorded in all patients.The laboratory indexes before hemodialysis,such as prothrombin(PT),activated partial thromboplastin time(APTT)and C-reactive protein(CRP),and baseline data,including diabetes history and age,of all patients were counted.The influencing factors of thrombosis in patients with long-term catheterization for hemodialysis were analyzed by logistic regression analysis.Results Within 6 months of catheterization,21(19.44%)of 108 patients with long-term catheterization for hemodialysis experienced thrombosis,and 87 cases(80.56%)were without thrombosis.Compared with the non-occurrence group,the occurrence group had elder age and higher levels of PT,APTT,CRP,moreover,the occurrence group had a higher proportion of diabetes compared with the non-occurrence group(P<0.05).Logistic regression analysis revealed that elder age,history of diabetes,high PT level,high APTT level,and high CRP level were the influencing factors of thrombosis in patients with long-term catheterization for hemodialysis(P<0.05).Conclusions Elder age,history of diabetes,high PT level,high APTT level,and high CRP level are the influencing factors of thrombosis in patients with long-term catheterization for hemodialysis.
