多普勒超声心动图对叁期职业性尘肺患者肺动脉压的估测价值

Value of Doppler echocardiography in estimating pulmonary artery pressure in patients with stageⅢoccupational pneumoconiosis

摘要 目的探讨多普勒超声心动图(UCG)对叁期职业性尘肺患者肺动脉压的估测价值。方法回顾性抽取2022年3月至2023年10月河南省第三人民医院收治的叁期职业性尘肺患者67例作为尘肺组,并以1∶1配比抽取同期健康体检者67例作为健康组,均采用UCG检查。对比两组右室内径(RV)、右室流出道内径(RVOTD)、右室流出道血流速度时间积分(VTI RVOT)、三尖瓣反流速度峰值(TRV)、肺动脉压、右心输出量(RCO)、三尖瓣环收缩期位移(TAPSE)。结果尘肺组肺动脉收缩压、肺血管阻力高于健康组(P<0.05);尘肺组RVOTD、TRV高于健康组,VTI RVOT低于健康组(P<0.05);尘肺组RCO、TAPSE低于健康组,RV高于健康组(P<0.05)。结论UCG应用于叁期职业性尘肺患者中,能评估肺动脉压力和肺血管阻力情况,监测患者右心功能,且不会对人体造成创伤。 Objective To investigate the value of Doppler echocardiography(UCG)in estimating pulmonary artery pressure in patients with stageⅢoccupational pneumoconiosis.Methods A total of 67 patients with stageⅢoccupational pneumoconiosis treated in Henan Provincial Third People’s Hospital from March 2022 to October 2023 were retrospectively selected as the pneumoconiosis group,and 67 healthy individuals hospitalized in the same period were selected as the health group in a 1∶1 ratio.All selected patients underwent UCG examination.The right ventricular diameter(RV),right ventricular outflow tract diameter(RVOTD),velocity time integral of right ventricular outflow tract(VTI RVOT),tricuspid regurgitation velocity(TRV),pulmonary artery pressure,right cardiac output(RCO),and tricuspid annular plane systolic excursion(TAPSE)were compared between the two groups.Results The pulmonary artery systolic pressure and pulmonary vascular resistance in the pneumoconiosis group were higher than those in the healthy group(P<0.05);RVOTD and TRV in the pneumoconiosis group were higher than those in the healthy group,while VTI RVOT was lower than that in the healthy group(P<0.05).The RCO and TAPSE in the pneumoconiosis group were lower than those in the healthy group,while RV was higher than that in the healthy group(P<0.05).Conclusions The application of UCG on stageⅢoccupational pneumoconiosis patients can evaluate pulmonary artery pressure and pulmonary vascular resistance,and monitor right heart function,without impact on human body.
