强力定眩片联合盐酸倍他司汀注射液治疗后循环缺血性眩晕的效果

Therapeutic effect of Qiangli Dingxuan tablets combined with betahistine hydrochloride injection on posterior circulation ischemic vertigo

目的分析强力定眩片联合盐酸倍他司汀注射液治疗后循环缺血性眩晕的效果。方法队列研究。抽取2021年6月至2022年12月漯河医学高等专科学校第三附属医院收治的后循环缺血性眩晕患者90例,按照随机数字表法分为单一组和联合组,每组45例。单一组予以盐酸倍他司汀注射液治疗,联合组在单一组的治疗基础上联合强力定眩片治疗。比较两组临床疗效、症状消失时间(眩晕、恶心呕吐、耳鸣、肢体/头面部麻木)及不良反应发生率;比较两组治疗前后眩晕程度[眩晕障碍调查量表(DHI)评分]、血流动力参数(基底动脉、右椎动脉、左椎动脉)水平、血液流变学(全血低切黏度、血浆纤维蛋白原、血浆黏度)水平。结果联合组治疗总有效率(95.56%,43/45)高于单一组(75.56%,34/45),P<0.05。联合组眩晕、恶心呕吐、耳鸣、肢体/头面部麻木消失时间均短于单一组(P均<0.05)。治疗后1、2周,联合组DHI评分低于单一组(P<0.05)。治疗后,联合组基底动脉、右椎动脉、左椎动脉血流速度均高于单一组(P均<0.05)。治疗后,联合组全血低切黏度、血浆纤维蛋白原、血浆黏度水平均低于单一组(P均<0.05)。联合组不良反应发生率(4.44%,2/45)与单一组(8.89%,4/45)比较差异未见统计学意义(P>0.05)。结论强力定眩片联合盐酸倍他司汀注射液治疗后循环缺血性眩晕患者,可改善血液循环和血液流变学情况,减轻临床症状,且安全性良好。

Objective To analyze the effect of Qiangli Dingxuan tablets combined with betahistine hydrochloride injection in the treatment of posterior circulation ischemic vertigo.Methods Ninety patients with posterior circulation ischemic vertigo treated in the Third Affiliated Hospital of Luohe Medical College from June 2021 to December 2022 were selected for the cohort study.And they were divided into a single group and a combination group by random number table method,with 45 cases in each group.The single group was treated with betahistine hydrochloride injection,and the combination group was treated with Qiangli Dingxuan tablets based on the treatment of the single group.The clinical efficacy,disappearance time of symptoms(dizziness,nausea and vomiting,tinnitus,numbness of limbs/head and face),and incidence of adverse reactions were compared between the two groups.The vertigo degree assessed by dizziness handicap inventory(DHI)score,levels of hemodynamic parameters(basilar artery,right vertebral artery and left vertebral artery),and levels of hemorheology(whole blood low shear viscosity,plasma fibrinogen and plasma viscosity)were compared between the two groups before and after treatment.Results The total effective rate of the combination group(95.56%,43/45)was higher than that of the single group(75.56%,34/45),P<0.05.The disappearance time of dizziness,nausea and vomiting,tinnitus and numbness of limbs/head and face in the combination group were shorter than those in the single group(all P<0.05).The DHI score of the combination group was lower than that of the single group 1 and 2 weeks after treatment(P<0.05).After treatment,the blood flow velocity of basilar artery,right vertebral artery and left vertebral artery in the combination group were higher than those in the single group(all P<0.05).After treatment,the levels of whole blood low shear viscosity,plasma fibrinogen and plasma viscosity in the combination group were lower than those in the single group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the combination group(4.44%,2/45)and the single group(8.89%,4/45),P>0.05.Conclusions Qiangli Dingxuan tablets combined with betahistine hydrochloride injection can improve blood circulation and hemorheology in patients with post circulation ischemic vertigo,and alleviate clinical symptoms,with good safety.
