特应性皮炎患者报告结局测量工具的研究进展

Research Progress of Patient-Reported Outcomes Measures of Atopic Dermatitis

特应性皮炎(atopic dermatitis,AD)是一种慢性炎症性皮肤病,在非致命皮肤病中疾病负担位列第一。患者报告结局(patient-reported outcomes,PROs)是直接来自患者关于自身健康状况的主观报告,在特应性皮炎的全程管理中越来越受到关注和重视。PRO测量工具(PRO measures,PROMs)是一类标准化、结构化收集PROs的测量工具,旨在帮助医护人员准确评估患者的症状、功能、健康相关生活质量等。其中,测量疾病严重程度的PROMs,包括:以患者为导向的湿疹测量表(patient-oriented eczema measure,POEM)、AD控制工具(atopic dermatitis control tool,ADCT)、以患者为导向的特应性皮炎评分(the patient oriented-SCORAD,PO-SCORAD)、峰值瘙痒数字评定量表(peak pruritus numeric rating scale,PP-NRS);评估患者生活质量的PROMs,包括:皮肤病生活质量指数(dermatology life quality index,DLQI)、儿童皮肤病生活质量指数(children dermatology life quality index,CDLQI)、婴幼儿皮炎生活质量指数(the infants′dermatitis quality of life index,IDQOL)、儿童特应性皮炎影响量表(childhood atopic dermatitis impact scale,CADIS)、特应性皮炎生活质量指数(the quality of life index foRatopic dermatitis,QOLIAD)。本文就目前常用的特应性皮炎患者特异性PROMs进行综述,以期为我国医护人员选择合适的评估工具提供借鉴。

Atopic dermatitis(AD)is a chronic inflammatory skin disease with the highest disease burden among non-fatal dermatoses.Patient-reported outcome(PRO)is a subjective report directly from patients about their own health status,which has been paid more and more attention in the whole-process management of AD.PRO measures(PROMs)refer to a kind of measurement tools for collecting PROs in a standardized and structured way,aiming to help medical staff accurately evaluate patients′symptoms,functions,health-related quality of life,etc.Among them,PROMs that measure the severity of the disease include:patient-oriented eczema measure(POEM),atopic dermatitis control tool(ADCT),the patient oriented-SCORAD(PO-SCORAD),peak pruritus numeric rating scale(PP-NRS);PROMs for assessing patients′quality of life include:dermatology life quality index(DLQI),children dermatology life quality index(CDLQI),the infants′dermatitis quality of life index(IDQOL),childhood atopic dermatitis impact scale(CADIS),the quality of life index for atopic dermatitis(QOLIAD).This article summarizes the commonly used PROMs for AD patients,in order to provide reference for medical workers in China to choose the appropriate measurement instrument.
