鸦葱疱疹膏联合普瑞巴林治疗带状疱疹后遗神经痛的效果

Effect of Scorzonera Herpes Zoster Ointment combined with Pregabalin in the treatment of post-herpetic neuralgia

目的鸦葱疱疹膏联合普瑞巴林治疗带状疱疹后遗神经痛的效果。方法选取2021年6月至2023年6月解放军联勤保障部队第九八八医院皮肤科60例带状疱疹后遗神经痛患者,按照随机数字表法分为对照组与观察组,各30例。对照组口服普瑞巴林胶囊,观察组在此基础上给予鸦葱疱疹膏阿是穴贴敷,治疗时间4周。比较两组临床疗效,比较两组治疗前后简化Mc Gill疼痛量表(SF-MPQ)评分、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分、匹兹堡睡眠质量指数(PSQI)评分、炎症因子水平。结果治疗4周后,观察组临床疗效优于对照组,差异有统计学意义(P<0.05)。整体分析发现:两组视觉模拟评分法(VAS)、疼痛评估指数(PRI)、现时疼痛程度(PPI)评分组间比较、时间点比较及交互作用,差异均有统计学意义(P<0.05)。组内比较:治疗2、4周后,两组VAS、PRI、PPI评分低于治疗前,治疗4周后低于治疗2周后,差异有统计学意义(P<0.05);组间比较:观察组治疗2、4周后VAS、PRI、PPI评分均低于同时间点对照组,差异有统计学意义(P<0.05)。整体分析发现:两组HAMD、HAMA、PSQI评分组间比较、时间点比较及交互作用,差异均有统计学意义(P<0.05)。组内比较:治疗2、4周后,两组HAMD、HAMA、PSQI评分低于治疗前,治疗4周后低于治疗2周后,差异有统计学意义(P<0.05);组间比较:观察组治疗2、4周后HAMD、HAMA、PSQI评分均低于同时间点对照组,差异有统计学意义(P<0.05)。整体分析发现:两组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平组间比较、时间点比较及交互作用,差异均有统计学意义(P<0.05)。组内比较:治疗2、4周后,两组TNF-α、IL-6水平低于治疗前,治疗4周后低于治疗2周后,差异有统计学意义(P<0.05);组间比较:观察组治疗后2、4周后TNF-α、IL-6水平均低于同时间点对照组,差异有统计学意义(P<0.05)。结论鸦葱疱疹膏阿是穴贴敷联合普瑞巴林胶囊口服可有效缓解带状疱疹后遗神经痛疼痛症状,改善患者焦虑、抑郁症状,提高睡眠质量,降低血清炎症因子表达。

Objective To study the effect of Scorzonera Herpes Zoster Ointment combined with Pregabalin in the treatment of post-herpetic neuralgia.Methods A total of 60 patients with post-herpetic neuralgia in the Department of Dermatology,Logistic Support Forces of the Chinese People’s Liberation Army No.988 Hospital from June 2021 to June 2023 were selected and they were divided into control group and observation group according to random number table method,with 30 cases in each group.The control group was given Pregabalin Capsules oral,and the observation group was treated with Scorzonera Herpes Zoster Ointment at ashi point application for four weeks.The clinical efficacy of the two groups was compared.The short-form McGill pain questionnaire(SF-MPQ)scores,Hamilton depression scale(HAMD)scores,Hamilton anxiety scale(HAMA)scores,Pittsburgh sleep quality index(PSQI)scores,and inflammatory factor levels of the two groups were compared before and after treatment.Results After four weeks of treatment,the clinical effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The overall analysis showed that there were statistically significant differences between the two groups in visual analogue scale(VAS),pain rating index(PRI),present pain intensity(PPI)scores between groups,time point comparison,and interaction(P<0.05).Intra-group comparison:after two and four weeks of treatment,VAS,PRI,and PPI scores of the two groups were lower than before treatment,and those after four weeks of treatment were lower than those after two weeks of treatment,and the differences were statistically significant(P<0.05).Comparison between groups:the VAS,PRI,and PPI scores of the observation group were lower than those of the control group after two and four weeks of treatment,and the differences were statistically significant(P<0.05).The overall analysis showed that there were statistically significant differences between the two groups in HAMD,HAMA,and PSQI scores between groups,time point comparison,and interaction(P<0.05).Intra-group comparison:after two and four weeks of treatment,HAMD,HAMA,and PSQI scores of the two groups were lower than before treatment,and those after four weeks of treatment were lower than those after two weeks of treatment,and the differences were statistically significant(P<0.05).Comparison between groups:HAMD,HAMA,and PSQI scores in the observation group were lower than those in the control group after two and four weeks of treatment,and the differences were statistically significant(P<0.05).The overall analysis showed that there were significant differences between the two groups in the levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)between groups,time point comparison,and interaction(P<0.05).Intra-group comparison:after two and four weeks of treatment,the levels of TNF-αand IL-6 in two groups were lower than before treatment,and those after four weeks of treatment were lower than those after two weeks of treatment,and the differences were statistically significant(P<0.05).Comparison between groups:the levels of TNF-αand IL-6 in observation group were lower than those in control group after two and four weeks of treatment,and the differences were statistically significant(P<0.05).Conclusion The Scorzonera Herpes Zoster Ointment at ashi point application combined with Pregabalin Capsules can effectively relieve the pain symptoms of post-herpetic neuralgia,improve the symptoms of anxiety and depression,improve the quality of sleep,and reduce the expression of serum inflammatory factors.
