摘要
目的 了解早产儿生后7 d甲状腺功能异常的发病率并分析其影响因素。方法 回顾性分析2023年3月至8月出生并入住山西医科大学附属山西省儿童医院新生儿内科261例早产儿的临床资料。根据生后7 d甲状腺功能检查结果将其分为正常组和异常组。分析早产儿生后7 d甲状腺功能异常的影响因素。结果 261例早产儿中甲状腺功能异常53例(20.3%)。两组胎龄、出生体重、分娩方式、妊娠高血压、使用肺表面活性物质、使用脂肪乳剂、合并动脉导管未闭比较,差异有统计学意义(P<0.05)。胎龄(OR=0.187)、分娩方式(OR=0.197)、合并动脉导管未闭(OR=0.251)是早产儿生后7 d甲状腺功能异常的影响因素(P<0.05)。结论 临床应对有高危因素早产儿的甲状腺功能进行密切监测,从而达到早发现、早诊断、早治疗,改善预后。
Objective To investigate the incidence of abnormal thyroid function seven days after birth of preterm infants,and to analyze the influencing factors.Methods Clinical data of 261 preterm infants born and admitted to the Department of Neonatology,Shanxi Children’s Hospital,Shanxi Medical University from March to August 2023 were retrospectively analyzed.According to the results of thyroid function seven days after birth,they were divided into normal group and abnormal group.The influencing factors of abnormal thyroid function seven days after birth of preterm infants were analyzed.Results Among 261 preterm infants,53(20.3%)had abnormal thyroid function.There were statistically significant differences in gestational age,birth weight,delivery mode,gestational hypertension,use of pulmonary surfactant,use of fat emulsion,and combined patent ductus arteriosus(PDA)between two groups(P<0.05).Gestational age(OR=0.187),delivery mode(OR=0.197),and combined patent ductus arteriosus(OR=0.251)were the influencing factors of abnormal thyroid function seven days after birth of preterm infants(P<0.05).Conclusion Thyroid function should be closely monitored clinically in preterm infants with high-risk factors so as to achieve early detection,diagnosis,and treatment,and to improve prognosis.
