摘要
病理性妊娠下血是指妇女妊娠期出现阴道出血的症状,可伴有腹痛、腰痛等症状。需及时治疗,否则血尽子死,胎堕难留。本文挖掘先秦时期至明清时期妊娠下血的相关记载,结合现代医家思想,探析医家临证观点,追溯理论来源,对各家学术思想及治法方药进行系统梳理,提出半产夹瘀论、风伤血论、胎前火热论、血气虚损论、肾虚论等病因病机,分别确立活血化瘀、疏风升肝、清热养血、益气摄血、补肾安胎等治法,并产生胶艾汤、助气补漏汤、寿胎丸等方剂和荆芥、防风、生地、黄芩、白术等用药,上述研究结果可为现代临床治疗病理性妊娠下血提供指导。
Pathological pregnancy bleeding refers to symptoms of vaginal bleeding during pregnancy,it can be accompanied by symptoms such as abdominal pain and low back pain.It needs to be treated in time,otherwise child will die from bleeding and abortion will be difficult to survive.This article explores relevant records of pregnancy bleeding from Pre Qin dynasty to Ming and Qing dynasties,combined with modern medical ideas,analyzes clinical views of medical practitioners,traces theoretical sources,and systematically sorts out academic ideas and treatment methods and medicines of various schools,proposes etiology and pathogenesis such as theories of semi-production with blood stasis,wind injury to blood,prenatal heat,blood deficiency and kidney deficiency,respectively establishes treatment methods of promoting blood circulation to remove blood stasis,dispersing wind to promote liver function,clearing heat to nourish blood,supplementing qi to control blood flow and tonifying kidney to prevent miscarriage,and produces prescriptions such as Jiaoai Decoction,Zhuqi Bulou Decoction,Shoutai Pills,and herbs such as Schizonepetae Herba,Saposhnikoviae Radix,Rehmanniae Radix,Scutellariae Radix,Atractylodis Macrocephalae Rhizoma.Above research results can provide guidance for modern clinical treatment of pathological pregnancy bleeding.
