经胸微创室间隔缺损封堵术临床分析

Clinical Analysis of Transthoracic Minimally Invasive Ventricular Septal DefectClosure

目的观察分析经胸微创室间隔缺损封堵术的应用效果。方法回顾性选取2018年2月至2023年12月期间在贵州医科大学附属医院接受经胸微创室间隔缺损封堵术的100例患者,均为单发,收集其临床资料,分析封堵术效果。结果100例患者中97例成功封堵,成功率97%(P<0.001),3例患者调整为低温体外循环手术。封堵成功的97例中41例使用偏心封堵器,56例使用等边封堵器,使用等边型封堵器类型的患者例数更多(P<0.05);随访3~6个月后,1例患者出现残余分流情况,更换封堵伞型号后再次封堵,分流情况消失。结论采用经胸微创室间隔缺损封堵手术有较高的封堵成功率,患者并发症发病率相对较低,可在临床中推广应用。

Objective To observe and analyze the effect of minimally invasive trans-thoracic ventricular septal defect closure.Methods 100 patients who received minimally invasive transthoracic ventricular septal defect closure in the Affiliated Hospital of Guizhou Medical University from February 2018 to December 2023 were retrospectively selected,all with single defects.Their clinical data were collected,and the effect of closure was analyzed.Results 97 out of 100 patients were successfully closed,with a success rate of 97%(P<0.001).Three patients were switched to hypothermic cardiopulmonary bypass surgery.Among the 97 successfully closed patients,41 cases were treated witheccentric occluders,and 56 cases were treated with equilateral occluders.More patients were treated with equilateral occluders(P<0.05).After 3 to 6 months of follow-up,one patient experienced residual shunt,and the shunt disappeared after the occluder model was replaced and closedagain.Conclusion Minimally invasive transthoracicventricular septal defectclosure has a high success rate and a relatively low incidence of complications,which can be widely used in clinic.
