摘要
目的探讨儿童急性鼻窦炎采用丙酸倍氯米松鼻气雾剂联合改良鼻腔冲洗方案治疗的临床效果。方法选择2020年1月至2023年11月夏津县人民医院耳鼻喉科接收的130例儿童急性鼻窦炎患儿为研究对象,通过随机数字法分为两组,对照组与研究组,每组各65例。对照组使用丙酸倍氯米松气雾剂治疗,研究组在对照组基础上联合改良鼻腔冲洗方案进行医治。观察各组治疗前后炎症因子白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素-8(Interleukin-8,IL-8)、感染指标降钙素原(Procalcitonin,PCT)、C反应蛋白(C-reactive protein,CRP)及临床疗效与不良反应。结果治疗前,两组患儿IL-6、IL-8、TNF-α、PCT、CRP水平无显著差异(P>0.05);治疗后,研究组IL-6、IL-8、TNF-α、PCT与CRP水平均显著低于对照组(P<0.05)。研究组临床总有效率为96.92%,显著高于对照组的76.92%,差异有统计学意义(P<0.05);研究组不良反应发生率为3.08%,显著低于对照组的13.85%,差异有统计学意义(P<0.05)。结论急性鼻窦炎患儿通过丙酸倍氯米松鼻气雾剂与改良鼻腔冲洗方案联合治疗,不但能减轻患儿的炎性反应,降低患儿的感染指标,还可以提升患儿的临床疗效,减少不良反应,保障患儿治疗的安全性。
Objective To investigate the clinical effect of treating children with acute rhinosinusitis with the combination of beclomethasone propionate nasal aerosol and modified nasal irrigation regimen.Methods 130 pediatric patients with acute rhinosinusitis admitted to the Department of Otolaryngology of Xiajin County People’s Hospital from January 2020 to November 2023 were selected and randomly divided into the study group(beclomethasone propionate nasal aerosol)and the control group(beclomethasone propionate nasal aerosol and modified nasal irrigation regimen),with 65 cases in each group.Inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-αα(TNF-α),interleukin-8(IL-8)]and infection markers[procalcitonin(PCT)and C-reactive protein(CRP)]were observed before and after treatment in each group,and the clinical efficacy and adverse reactions were analyzed.Results There was no significant difference in the levels of IL-6,IL-8,TNF-α,PCT,and CRP between the two groups before treatment(P>0.05).After treatment,the levels of IL-6,IL-8,TNF-α,PCT,and CRP in the study group were significantly lower than those in the control group(P<0.05).The total clinical effective rate of the study group was 96.92%,which was significantly higher than that of the control group(76.92%),and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was 3.08%,which was significantly lower than that in the control group(13.85%),and the difference was statistically significant(P<0.05).Conclusion Treating patients with acute sinusitis with the combination of beclomethasone propionate nasal aerosol and modified nasal irrigation regimen can not only reduce the inflammatory response and the infection index,but also improve the clinical efficacy,reduce adverse reactions,and ensure the treatment safety of patients.
