miRNA-1-3p表达水平与老年慢性心力衰竭的相关性

摘要 目的:探讨miRNA-1-3p表达水平与老年慢性心力衰竭(CHF)的相关性。方法:选择我院2019年6月~2021年8月收治的年龄≥65岁的老年CHF住院患者45例为CHF组,同期体检的年龄匹配的健康老人45例为对照组。行心脏超声测定左室舒张末内径(LVEDd)和左室射血分数(LVEF)、测定血浆脑钠肽(BNP)水平;采用实时荧光定量逆转录聚合酶链反应(qRT-PCR)方法测定两组外周血单核细胞miRNA-1-3p表达水平。比较两组上述指标水平。结果:与对照组相比,CHF组血浆BNP[(38.34±23.02)pg/ml比(679.89±338.72)pg/ml]水平、LVEDd[45(42,47)mm比50(46,54)mm]和miRNA-1-3p表达[1.09(0.68,1.62)比1.42(0.98,2.35)]显著升高,而LVEF[(56.86±3.97)%比(48.89±8.32)%]显著降低,P均<0.01。Spearman相关分析显示血浆BNP、LVEDd与miRNA-1-3p表达呈显著正相关(r=0.303、0.223,P=0.006、0.049),LVEF与miRNA-1-3p表达呈显著负相关(r=-0.235,P=0.047)。结论:外周血单核细胞miRNA-1-3p有可能作为诊断老年慢性心力衰竭的一种新型标志物及治疗靶点。 Objective:To investigate the correlation between miRNA-1-3p expression level and chronic heart failure(CHF)in the elderly.Methods:A total of 45 elderly CHF inpatients≥65 years who were treated in our hospital from June 2019 to August 2021 were selected as CHF group.Another 45 healthy elderly people with corresponding age who received physical examination simultaneously were selected as control group.Left ventricular end diastolic diameter(LVEDd)and left ventricular ejection fraction(LVEF)were measured by cardiac ultrasound,and plasma brain natriuretic peptide(BNP)level was measured.The expression levels of miRNA-1-3p in peripheral blood monocytes of the two groups were determined by real-time fluorescence quantitative reverse transcription polymerase chain reaction(qRT-PCR).Levels of above indexes were compared between two groups.Results:Compared with control group,there were significant rise in plasma BNP level[(38.34±23.02)pg/ml vs.(679.89±338.72)pg/ml],LVEDd[45(42,47)mm vs.50(46,54)mm]and miRNA-1-3p expression[1.09(0.68,1.62)vs.1.42(0.98,2.35)],and significant reduction in LVEF[(56.86±3.97)%vs.(48.89±8.32)%]in CHF group,P<0.01 all.Spearman correlation analysis indicated that plasma BNP and LVEDd were significant positively correlated with miRNA-1-3p expression(r=0.303,0.223,P=0.006,0.049),while LVEF was significant inversely correlated with miRNA-1-3p expression(r=-0.235,P=0.047).Conclusion:miRNA-1-3p in peripheral blood monocytes may be a novel marker and therapeutic target for the diagnosis of chronic heart failure in the elderly.
