基于体素内不相干运动扩散加权成像多模态磁共振技术对高危人群早期小肝癌诊断价值

摘要 目的探讨基于体素内不相干运动扩散加权成像(IVIM-DWI)的多模态磁共振技术对高危人群早期小肝癌(sHCC)的诊断价值。方法选取自2020年6月至2023年12月联勤保障部队第九〇六医院收治的50例高危人群sHCC患者为研究对象。所有患者均于术前行3.0 T磁共振成像常规、普美显增强、IVIM-DWI、磁敏感加权成像(SWI)扫描。主要测量参数:DWI单指数成像表观扩散系数(ADC)值;IVIM-DWI成像参数包括真性扩散系数(D)值、灌注分数(f)值和假性扩散系数(D*)值。采用受试者工作特征(ROC)曲线,根据曲线下面积(AUC)分析比较各参数的诊断效能。采用Spearman相关性分析各参数与病理分级的相关性。分析研究IVIM-DWI量化参数值及多模态磁共振技术对sHCC的检出率。结果ADC值和D值诊断sHCC的AUC值分别为0.858、0.820,具有的一定的诊断价值;两者联合诊断sHCC的AUC值为0.872;联合诊断的AUC值大于单一参数检测,差异有统计学意义(P<0.05)。不同sHCC病理分级患者ADC值、D值、f值和D*值比较,差异均有统计学意义(P<0.05)。Spearman相关性分析发现,ADC值、D值与sHCC病理分级呈负相关(r=-0.495、-0.472,P<0.05)。普美显磁共振增强检查检出全部病灶,正确定性诊断45例。结论基于IVIM-DWI的多模态磁共振技术能够有效提高高危人群的sHCC早期检出率。IVIM-DWI参数均可量化,其中,D值、ADC值及两者联合应用均具有较高的诊断效能,提高了sHCC的诊断灵敏度及特异度,且可以进行肿瘤初步病理分级。
