足月胎膜早破孕妇脐带血IGFBP-3、IGFBP-5水平与合并宫内感染的关系

The relationship between umbilical cord blood IGFBP-3,IGFBP-5 levels and intrauterine infection in pregnant women with premature rupture of membranes

目的探讨足月胎膜早破孕妇脐带血胰岛素生长因子结合蛋白-3(IGFBP-3)、胰岛素样生长因子结合蛋白-5(IGFBP-5)水平与合并宫内感染的关系。方法选取80例足月胎膜早破孕妇,根据分娩后是否发生宫内感染分为感染组(40例)与未感染组(40例)。比较两组脐带血IGFBP-3、IGFBP-5水平并评估预测价值。结果感染组脐带血IGFBP-3、IGFBP-5水平均高于未感染组(P<0.05)。脐带血IGFBP-3、IGFBP-5联合预测的AUC为0.908。多因素Logistic回归分析显示:IGFBP-3、IGFBP-5、破膜至分时间(OR=2.523,95%CI:1.676,3.788)均是影响足月胎膜早破孕妇合并宫内感染的危险因素(OR>1)(P<0.05)。结论足月胎膜早破合并宫内感染孕妇脐带血IGFBP-3、IGFBP-5水平呈高表达,两者可作为诊断足月胎膜早破孕妇宫内感染的临床指标,联合预测诊断效能更高。

Objective To investigate the relationship between the levels of insulin growth factor binding protein-3(IGFBP-3)and insulin-like growth factor binding protein-5(IGFBP-5)in umbilical cord blood and intrauterine infection in pregnant women with premature rupture of membranes.Methods 80 pregnant women with premature rupture of membranes were selected and divided into infected group(40 cases)and uninfected group(40 cases)according to whether intrauterine infection occurred after delivery.The levels of IGFBP-3 and IGFBP-5 in cord blood of the two groups were compared and the predictive value was evaluated.Results The levels of IGFBP-3 and IGFBP-5 in cord blood of infected group were higher than those of uninfected group(P<0.05).The combined prediction AUC of IGFBP-3 and IGFBP-5 in cord blood was 0.908.Multivariate Logistic regression analysis showed that IGFBP-3,IGFBP-5 and the time from membrane rupture to delivery(OR=2.523,95%CI:1.676,3.788)were all risk factors for intrauterine infection in pregnant women with term premature rupture of membranes(OR>1)(P<0.05).Conclusion The levels of IGFBP-3 and IGFBP-5 in the cord blood of pregnant women with PROM complicated with intrauterine infection were highly expressed,and they can be used as clinical indicators for the diagnosis of intrauterine infection,and the combined prediction has higher diagnostic efficacy.
