血清、腹腔液AMH水平对子宫内膜异位症合并不孕症患者术后妊娠结局的影响

Effect of serum and peritoneal fluid AMH levels on postoperative pregnancy outcomes in patients with endometriosis-related infertility

目的探讨血清、腹腔液抗苗勒管激素(AMH)水平对子宫内膜异位症(EAI)合并不孕症患者术后妊娠结局的临床预测价值。方法选取122例行宫腹腔镜联合诊治术的患者,患者通过手术病理确诊为不孕症,按照前瞻性研究方法将患者分为试验组(子宫内膜异位症合并不孕症患者,EAI组)和对照组(非子宫内膜异位症合并不孕症患者,Non-EAI组),每组61例。所有患者均在月经干净后第3~7天进行手术。比较两组血清、腹腔液AMH水平和术后妊娠率。结果EAI组的血清、腹腔液AMH水平均显著低于Non-EAI组,差异有统计学意义(P<0.05)。EAI组的术后妊娠率提高,与Non-EAI组对比,差异无统计学意义(P>0.05)。结论血清、腹腔液AMH水平在预测子宫内膜异位症合并不孕症患者手术治疗后的妊娠结局方面具有重要的临床价值。

Objective To investigate the clinical value of anti-Mullerian hormone(AMH)levels in serum and peritoneal fluid in predicting pregnancy outcomes in patients with endometriosis(EAl)complicated with infertility.Methods 122 patients undergoing combined uterine laparoscopic diagnosis and treatment were selected,and the patients were diagnosed with infertility through surgical pathology.According to prospective research methods,the patients were divided into experimental group(patients with endometriosis complicated with infertility,EAI group)and control group(patients with non-endometriosis complicated with infertility,Non-EAI group),with 61 cases in each group.All patients underwent surgery on the 3rd to 7th day after menstruation had cleared.AMH levels in serum and peritoneal fluid and pregnancy rate were compared between the two groups.Results AMH levels in serum and peritoneal fluid in EAI group were significantly lower than those in Non-EAI group,with statistical significance(P<0.05).The postoperative pregnancy rate of EAI group was higher than that of Non-EAI group,and the difference was not statistically significant(P>0.05).Conclusion The level of AMH in serum and peritoneal fluid has important clinical value in predicting the pregnancy outcome of endometriosis patients with infertility after surgical treatment.
