血府逐瘀汤治疗经皮冠状动脉介入术术后心绞痛患者的临床研究

Clinical Research of Xuefu Zhuyu Decoction(血府逐瘀汤)in Treatment of Patients with Angina Pectoris after PCI

目的分析血府逐瘀汤治疗经皮冠状动脉介入术(percutaneous coronary intervention,PCI)术后心绞痛患者的治疗效果。方法2019年6月—2022年6月山西省长治市中医研究所附属医院心病科门诊和住院收治的PCI术后心绞痛患者100例,通过随机数字表法分为研究组50例及对照组50例。对照组接受常规治疗,研究组在对照组基础上接受血府逐瘀汤治疗,两组均治疗1个月。比较两组治疗后疗效,治疗前、后中医证候积分、心功能指标、心绞痛发作情况及生活质量。结果治疗后,研究组总有效率高于对照组[94.00%(47/50)vs 74.00%(37/50),P<0.05]。治疗后,两组发作次数、疼痛持续时间、硝酸甘油日消耗量,胸闷、胸痛、心悸气短、倦怠乏力、面色紫黯评分较治疗前降低,且研究组更低(P<0.05)。治疗后,两组西雅图心绞痛量表(Seattle Angina Scale,SAQ)评分、左室射血分数(left ventricular ejection fraction,LVEF)、每搏输出量(stroke volume,SV)、6 min步行试验距离较治疗前升高,且研究组更高,差异有统计学意义(P<0.05)。结论血府逐瘀汤在PCI术后心绞痛中应用效果确切,能降低中医证候得分,控制心绞痛发作情况,优化心功能,最终改善生活质量。

Objective To analyze the therapeutic effect of Xuefu Zhuyu Decoction(血府逐瘀汤)in the treatment of patients with angina pectoris after percutaneous coronary intervention(PCI).Methods From June 2019 to June 2022,100 patients with angina pectoris after PCI admitted to the Department of Cardiology of Affiliated Hospital of Changzhi Institute of Traditional Chinese Medicine in Shanxi Province were divided into the study group(50 cases)and the control group(50 cases)by random number table method.The control group received conventional treatment,and the study group received Xuefu Zhuyu Decoction on the basis of the control group.Both groups were treated for 1 month.The therapeutic effect after treatment,traditional Chinese medicine syndrome scores,cardiac function indexes,angina attack and quality of life were compared between the two groups before and after treatment.Results After treatment,the total curative effect in the study group was higher[94.00%(47/50)vs 74.00%(37/50),P<0.05].After treatment,the number of attacks,duration of pain,daily consumption of nitroglycerin,chest tightness,chest colic,palpitation,shortness of breath,fatigue and dark complexion in the two groups were significantly lower than those before treatment,and the scores were lower in the study group than those before treatment(P<0.05).After treatment,the score of Seattle Angina Scale(SAQ),the values of left ventricular ejection fraction(LVEF),stroke volume(SV)and 6-minute walk test distance in the two groups increased,and those were higher in the study group(P<0.05).Conclusion Xuefu Zhuyu Decoction was effective in the treatment of patients with angina pectoris after PCI,and could improve the traditional Chinese medicine syndrome and the heart function and control the angina pectoris,and ultimately improves the quality of life.
