深刺纳阳针法与常规针刺疗法分别联合生物反馈电刺激治疗老年缺血性脑卒中并发尿失禁对膀胱最大容量的影响

Effects of Deep Receiving Yang Acupuncture and Routine Acupuncture Combined with Biofeedback Electrical Stimulation on Maximum Bladder Capacity in Patients with Cerebral Ischemic Stroke and Urinary Incontinence

目的探讨深刺纳阳针法与常规针刺疗法分别联合生物反馈电刺激治疗老年缺血性脑卒中(cerebral ischemic stroke,CIS)并发尿失禁对膀胱最大容量的影响。方法选取2021年10月—2022年10月诊治的138例老年CIS并发尿失禁患者,按照随机数表法分为观察组69例(深刺纳阳针法+生物反馈电刺激治疗)和对照组69例(常规针刺疗法+生物反馈电刺激治疗),观察两组排尿日记指标、尿流动力学、尿失禁程度及不良反应。结果治疗后,观察组单次尿量、排尿间隔时间高于对照组,48 h排尿次数、48 h漏尿次数显著低于对照组(P<0.05);治疗后,观察组膀胱最大容量高于对照组,最大逼尿肌压、残余尿量低于对照组(P<0.05);观察组尿失禁程度低于对照组(P<0.05);两组不良反应比较,差异无统计学意义(P>0.05)。结论深刺纳阳针法配合生物反馈电刺激治疗CIS并发尿失禁的疗效显著,且优于常规针刺疗法联合生物反馈电刺激治疗,能够有效改善排尿症状、尿流动力学以及神经功能缺损,且安全性较好。

Objective To explore the effects of deep receiving Yang acupuncture and routine acupuncture combined with biofeedback electrical stimulation on maximum bladder capacity in patients with cerebral ischemic stroke(CIS)and urinary incontinence.Methods A total of 138 patients with CIS and urinary incontinencewere enrolled as the research objects between October 2021 and October 2022.According to random number table method,they were divided into observation group(69 cases,deep receiving Yang acupuncture combined with biofeedback electrical stimulation)and control group(69 cases,routine acupuncture combined with biofeedback electrical stimulation).The urination diary indexes,urodynamics,urinary incontinence and adverse reactions in both groups were observed.Results After treatment,once urine volume and urination interval in the observation group were higher than those in the control group,and the times of 48 h urination and urine leakage were lower than those in the control group(P<0.05).After treatment,the maximum bladder capacity in the observation group was larger than that in the control group,the maximum detrusor pressure and residual urine volume were lower than those in the control group(P<0.05).After treatment,the severity of urinary incontinence in the observation group was significantly milder than that in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The deep receiving Yang acupuncture combined with biofeedback electrical stimulation has significant effect on CIS combined with urinary incontinence,which is superior to the routine acupuncture combined with biofeedback electrical stimulation and can effectively improve urinationsymptoms,urodynamics and neurological deficits,with higher safety.
