呼吸功能训练对肺癌根治术患者肺功能恢复和生活质量的影响

Effect of Respiratory Function Training on the Lung Function and Quality of Life in Patients Undergoing Radical Resection of Lung Cancer

目的探讨呼吸功能训练对肺癌根治术患者肺功能和生活质量的影响。方法将2021年1月至2022年12月在河南省胸科医院行胸腔镜下肺癌根治术的98例患者随机分为观察组和对照组,各49例。对照组接受常规护理,观察组在此基础上接受呼吸功能训练。通过第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、每分钟最大通气量(MVV)和呼气流量峰值(PEF)评价肺功能,记录两组患者术后恢复时间,采用癌症患者生命质量测定量表(QLQ-C30)进行生活质量评价。结果术后7 d时,两组患者FEV_(1)、FVC、MVV和PEF等肺功能指标均降低,但观察组各指标均高于对照组(P<0.05)。观察组患者术后呼吸机使用时间、置管时间、住院时间短于对照组(P<0.05)。术后1个月时,两组患者QLQ-C30评分升高,且观察组高于对照组(P<0.05)。结论呼吸功能训练能够促进肺癌根治术患者肺功能恢复,缩短术后康复时间,提高生活质量。

Objective To explore the effect of respiratory function training on the lung function and quality of life in patients undergoing radical resection of lung cancer.Methods A total of 98 patients who underwent thoracoscopic radical resection of lung cancer in Henan Chest Hospital from January 2021 to December 2022 were selected and randomly divided into observation group and control group,with 49 cases in both groups.During perioperative period,the control group was given routine nursing intervention,and the observation group was given respiratory function training on this basis.The forced expiratory volume in first second(FEV_(1)),forced vital capacity(FVC),maximum ventilatory volume(MVV)and peak expiratory flow(PEF)were used to evaluate lung function,the postoperative recovery time of the two groups was recorded,and cancer-specific quality of life questionnaire-core 30(QLQ-C30)was used to evaluate the quality of life of cancer patients.Results Seven days after surgery,the lung function indexes of FEV_(1),FVC,MVV and PEF were decreased in both groups,but the indexes in the observation group were significantly higher than those in the control group(P<0.05).The time of ventilator use,catheterization and hospitalization in the observation group were significantly lower than those in the control group(P<0.05).At 1 month after surgery,the scores of functional,symptom and overall health in QLQ-C30 were improved in both groups,and the fields scores in the observation group were higher than those in the control group(P<0.05).Conclusion Perioperative respiratory function training could effectively promote the recovery of lung function in patients undergoing radical resection of lung cancer,shorten the postoperative rehabilitation time,improved the quality of life.
