八段锦运动疗法对肩周炎临床疗效的荟萃分析

Meta analysis of the clinical effect of Baduanjin exercise therapy in the treatment of scapulohumeral periarthritis

目的:系统评价八段锦运动治疗肩周炎的临床疗效。方法:计算机检索中国知网(CNKI)、维普资讯网(VIP)和万方数据知识服务平台、中国生物医学文献服务系统、考克兰图书馆(Cochrane Library)、PubMed医学文献检索服务系统、Embase及Web of Science等中英文数据库,纳入有关八段锦治疗肩周炎的随机对照试验(RCT)。以Cochrane偏倚风险评估工具进行质量评价,采用RevMan 5.3统计软件进行荟萃分析。结果:共纳入9篇文献752例患者。荟萃分析结果显示,单纯或联合八段锦的试验组临床有效率较对照组高[OR=4.05,95%CI(2.15,7.63),P<0.0001];VAS评分[MD=-1.05,95%CI(-1.92,-0.18),P=0.02]、Melle评分[MD=-0.88,95%CI(-1.24,-0.52),P<0.00001]、Constant-Murley评分[MD=1.21,95%CI(0.44,1.97),P=0.002],均显著优于对照组。敏感性分析显示结果稳定可靠。结论:当前有限证据表明,八段锦运动能有效改善肩周炎患者的临床症状,但仍需要更多的高质量的临床研究去验证。

Objective:To systematically evaluate the clinical efficacy of Baduanjin in the treatment of scapulohumeral periarthritis.Methods:Computer retrieval of CNKI(CNKI),VIP Information Network(VIP),Wanfang data knowledge service platform,China biomedical literature service system,Cochrane Library,Medical literature retrieval service system(PubMed),Embase,and Web of Science were searched by the computer,and the randomized controlled trial(RCT)of Baduanjin in the treatment of scapulohumeral periarthritis was included.The Cochrane bias risk assessment tool was used for quality evaluation,and RevMan 5.3 statistical software was used for Meta analysis.Results:Nine references were finally included with a total of 752 patients.Meta analysis results showed that compared with the control group,the clinical effective rate in the experimental group with only or in combination with Baduanjin was higher than that in the control group[OR=4.05,95%CI(2.15,7.63),P<0.0001];the VAS score[MD=-1.05,95%CI(-1.92,-0.18),P=0.02],Melle score[MD=-0.88,95%CI(-1.24,-0.52),P<0.00001],Constant-Murley score[MD=1.21,95%CI(0.44,1.97),P=0.002]were significantly better than those in the control group.Sensitivity analysis showed that the results were stable and reliable.Conclusion:Current limited evidence suggests that Baduanjin exercise can effectively improve the clinical symptoms of patients with scapulohumeral periarthritis,however,more high-quality clinical researches are still needed to verify in the future.
