深吸气屏气技术在乳腺癌保乳术后容积旋转调强放疗中的剂量学研究

摘要 目的:研究容积旋转调强技术与深吸气屏气技术相结合在左侧乳腺癌保乳术后放疗中剂量学特点及临床应用价值。方法:选取2021年1月至2022年6月在甘肃省肿瘤医院进行放疗的40例左侧乳腺癌保乳术后女性患者,20例患者在深吸气屏气状态下进行放疗(DIBH-VMAT组),20例患者在自由呼吸状态下进行放疗(FB-VMAT组),比较两组患者两种呼吸模式下进行容积旋转调强放疗的剂量学差异。结果:根据两组患者医生勾画的靶区分别设计的DIBH-VMAT和FB-VMAT两种治疗计划,DIBH心脏V30、心脏V20、左肺V20受量体积均低于FB,但左肺V10、左肺V5、右肺V5的低剂量区DIBH比FB体积有所增大,差异有统计学意义(P<0.05);剂量学结果显示:心脏V30 Dmean、左肺V20 Dmean及靶区的适形度及靶区剂量均匀性DIBH明显优于FB,但左肺V5 Dmean、右肺V5 DmeanDIBH稍高于FB,且均在危机器官最低限量允许范围之内,差异有统计学意义(P<0.05)。结论:容积旋转调强技术和深吸气屏气技术相结合的临床应用,使左侧乳腺癌保乳术后放疗精准性明显提高,正常组织及危机器官的受照体积和剂量明显降低,减轻了心脏、左肺的毒副反应,减少呼吸运动带来的治疗误差,临床优势明显,值得推行。
