大容量全肺灌洗联合振动排痰治疗尘肺病的临床效果

Clinical Effect of Large Volume Lung Lavage Combined with Vibration Expectoration in the Treatment of Pneumoconiosis

目的:探究大容量全肺灌洗(WLL)联合振动排痰治疗尘肺病的临床效果。方法:选取2018年12月—2023年12月天水市第一人民医院收治的尘肺病患者70例为研究对象,根据随机数字表法分为参照组和联合组,各35例。参照组给予大容量WLL,联合组在对照组基础上给予联合振动排痰。比较两组患者症状改善时间、临床疗效、炎性因子及肺功能。结果:联合组发热、呼吸困难、咳嗽、排痰困难改善时间短于参照组,差异有统计学意义(P<0.05);联合组总有效率高于参照组,差异有统计学意义(P=0.034);治疗后,两组降钙素原、肿瘤坏死因子-α、白细胞介素-1β、C反应蛋白水平低于治疗前,联合组低于参照组,差异有统计学意义(P<0.05);治疗后,两组最大通气量、第1秒用力呼气容积、用力肺活量、最大呼气流量水平高于治疗前,联合组高于参照组,差异有统计学意义(P<0.05)。结论:大容量WLL联合振动排痰治疗尘肺病的效果显著,有助于改善患者临床症状及肺功能,降低炎性反应。

Objective:To explore the clinical effect of large volume lung lavage(WLL)combined with vibration expectoration on pneumoconiosis.Methods:A total of seventy pneumoconiosis patients admitted to Tianshui First People's Hospital from December 2018 to December 2023 were selected as the study objects,and were divided into the reference group and the combined group according to random number table method,with thirty-five cases in each group.The reference group was given large volume WLL,and the combined group was given combined vibration expectoration based on the reference group.Symptom improvement time,clinical efficacy,inflammatory factors and lung function were compared between the two groups.Results:The improvement time of fever,dyspnea,cough and expectoration difficulty in combined group was shorter than that in reference group,and the difference was statistically significant(P<0.05);the total effective rate of the combined group was higher than that of the reference group,and the difference was statistically significant(P=0.034);after treatment,the levels of procalcitonin,tumor necrosis factor-α,interleukin-1βand C-reactive protein in two groups were lower than before treatment,and those in combined group were lower than those in reference group,the difference was statistically significant(P<0.05);after treatment,the maximum volume of gas,forced expiratory volume in the first second,forced vital capacity and maximum expiratory flow in two groups were higher than before treatment,and the combined group was higher than the reference group,the difference was statistically significant(P<0.05).Conclusion:Large WLL combined with vibration expectoration is effective in the treatment of pneumoconiosis,which is helpful to improve clinical symptoms and lung function,and reduce inflammatory response.
