甲状腺乳头状癌患者组织中表皮生长因子受体、维生素D受体、溴结构域蛋白4表达水平及其临床意义

摘要 目的:探究甲状腺乳头状癌(PTC)患者组织中表皮生长因子受体(EGFR)、维生素D受体(VDR)、溴结构域蛋白4(BRD4)的表达水平和其临床意义。方法:选取2022年2月—2024年3月广州医科大学附属妇女儿童医疗中心增城院区收治的PTC患者54例作为研究对象,取患者的癌组织及癌旁正常组织,采用免疫组织化学技术检测组织中EGFR、VDR和BRD4蛋白表达水平,并分析EGFR、VDR、BRD4阳性表达与临床病理特征的关系。结果:PTC患者癌组织中EGFR、VDR及BRD4蛋白表达阳性率均高于癌旁正常组织,差异有统计学意义(P<0.001)。不同临床病理特征的EGFR阳性表达情况比较,差异无统计学意义(P>0.05);瘤体>1 cm、TNM分期Ⅲ~Ⅳ期者VDR阳性率高于瘤体≤1 cm、TNM分期Ⅰ~Ⅱ期者,差异有统计学意义(P<0.05);瘤体>1 cm、有淋巴结转移、TNM分期Ⅲ~Ⅳ期者BRD4阳性率高于瘤体≤1 cm、无淋巴结转移、TNM分期Ⅰ~Ⅱ期者,差异有统计学意义(P<0.05)。EGFR蛋白表达与VDR、BRD4蛋白表达呈正相关(r=0.462、0.513,P<0.001);VDR蛋白与BRD4蛋白表达呈正相关(r=0.407,P<0.001)。结论:EGFR、VDR和BRD4在PTC患者组织中呈显著高表达,与PTC的形成及临床病理特征有一定相关性,可作为治疗PTC的监测指标。 Objective:To investigate the expression levels of epidermal growth factor receptor(EGFR),vitamin D receptor(VDR)and bromine domain protein 4(BRD4)in the tissues of patients with thyroid papillary carcinoma(PTC)and their clinical significance.Methods:Fifty-four patients with PTC admitted to Zengcheng Branch of Women and Children's Medical Center Affiliated to Guangzhou Medical University from February 2022 to March 2024 were selected as the study objects.Cancerous tissue and adjacent normal tissue were taken from the patient.The protein expression levels of EGFR,VDR and BRD4 were detected by immunohistochemistry.The relationship between positive expression of EGFR,VDR and BRD4 and clinicopathological features was analyzed.Results:The expression positive rates of EGFR,VDR and BRD4 protein in cancer tissues of PTC patients were higher than those in adjacent normal tissues,and the difference was statistically significant(P<0.001).There was no significant difference in the expression of EGFR among different clinicopathological features(P>0.05);the positive rate of VDR in patients with tumor size>1 cm and TNM stageⅢ-Ⅳwas higher than that in patients with tumor size≤1 cm and TNM stageⅠ-Ⅱ,the difference was statistically significant(P<0.05);the positive rate of BRD4 in patients with tumor size>1 cm,lymph node metastasis and TNM stageⅢ-Ⅳwas higher than that in patients with tumor size≤1 cm,no lymph node metastasis and TNM stageⅠ-Ⅱ,the difference was statistically significant(P<0.05).EGFR protein expression was positively correlated with VDR and BRD4 protein expression(r=0.462,0.513,P<0.001);VDR protein was positively correlated with BRD4 protein expression(r=0.407,P<0.001).Conclusion:EGFR,VDR and BRD4 are significantly overexpressed in PTC patients,correlated with the formation and clinicopathological characteristics of PTC,and can be used as monitoring indicators for PTC treatment.
