改良式无痛翻身方案联合集束化康复护理应用于老年股骨颈骨折患者中的效果

The Effect of Improved Painless Tumbling Plan Combined with Bundled Rehabilitation Nursing on Elderly Patients with Femoral Neck Fractures

目的:探讨改良式无痛翻身方案联合集束化康复护理应用于老年股骨颈骨折患者的效果。方法:选取2021年1月—2023年1月惠州市中心人民医院收治的100例老年股骨颈骨折患者作为研究对象。按照随机数表法将其分为对照组(n=50)和观察组(n=50),其中对照组给予集束化康复护理干预,观察组给予改良式无痛翻身方案联合集束化康复护理干预。观察两组患者疼痛程度[视觉模拟评分(VAS)]、活动能力[功能独立性评定量表(FIM)、Harris髋关节评分、日常生活能力量表(Barthel指数)]、自尊感[自尊感评分(SES)]、压力性损伤(Braden压力性损害评分)情况。结果:观察组患者翻身前、翻身时、翻身完成即刻VAS评分均较对照组低,差异有统计学意义(P<0.05);干预后,观察组患者FIM、Harris评分、Barthel指数均较对照组高,差异有统计学意义(P<0.05);干预3 d后、干预7 d后,观察组患者SES评分均较对照组低,Braden压力性损伤评分较对照组高,差异有统计学意义(P<0.05)。结论:对老年股骨颈骨折患者采用改良式无痛翻身方案联合集束化康复护理可有效缓解其疼痛程度,提高髋关节活动能力、自尊感,减少压力性损伤的发生情况。

Objective:To explore the application of improved painless turning scheme combined with cluster rehabilitation nursing in elderly patients with femoral neck fracture.Method:A prospective study was conducted on 100 elderly patients with femoral neck fractures admitted to Huizhou Munieipal Central Hospital from January 2021 to January 2023.All patients were randomly divided into the control group and the observation group according to a random number table.Among them,50 patients of the control group,who received clustered rehabilitation care,and 50 patients of the observation group,who received modified painless turning program and clustered rehabilitation care.The pain levels[visual analog scale(VAS)],activity ability[functional independence measure(FIM),Harris hip score,activity of daily living scale(Barthel index)],self-esteem[self-esteem score(SES)],and pressure injury(Braden pressure injury scale)were observed in both groups.Result:The VAS scores of patients in the observation group before,during,and immediately after turning were lower than those in the control group,and the difference was statistically significant(P<0.05).After intervention,the FIM,Harris score,and Barthel index of patients in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).After 3 days and 7 days of intervention,the SES scores of patients in the observation group were lower than those in the control group,and the Braden pressure injury score was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The use of an improved painless roll over plan combined with a bundled rehabilitation nursing plan for elderly patients with femoral neck fractures,which can effectively alleviate their pain level,improve hip joint function,self-esteem,and daily living ability.
