^(125)I放射性粒子植入治疗对老年非小细胞肺癌患者癌性疼痛的影响

摘要 目的探讨^(125)I放射性粒子植入治疗对老年非小细胞肺癌患者癌性疼痛的影响。方法将110例老年非小细胞肺癌患者根据治疗方式的不同分为化疗组(n=54)和粒子植入组(n=56)。化疗组患者采取紫杉醇+顺铂化疗方案,粒子植入组患者在化疗组的基础上采取^(125)I放射性粒子植入治疗。比较两组患者的临床疗效、疼痛程度[视觉模拟评分法(VAS)]、衰弱情况[Fried衰弱表型量表(FFP)]、肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC]及不良反应发生情况。结果粒子植入组患者总有效率高于化疗组,差异有统计学意义(P﹤0.05)。治疗后,两组患者VAS、FFP评分均较治疗前降低,FVC、FEV1及FEV1/FVC均较治疗前升高,且粒子植入组患者VAS、FFP评分均低于化疗组,FVC、FEV1及FEV1/FVC均高于化疗组,差异均有统计学意义(P﹤0.05)。两组患者不良反应总发生率比较,差异无统计学意义(P﹥0.05)。结论^(125)I放射性粒子植入治疗老年非小细胞肺癌患者,可以扭转衰弱,抑制癌性疼痛,提高肺功能,且较为安全,疗效确切。 Objective To investigate the effect of ^(125)I radioactive particle implantation on cancer pain in elderly patients with non-small cell lung cancer.Method A total of 110 elderly patients with non-small cell lung cancer were divided into chemotherapy group(n=54)and particle implantation group(n=56).Patients in the chemotherapy group received paclitaxel+cisplatin chemotherapy regimen,and patients in the particle implantation group received ^(125)I radioactive particle implantation therapy based on the chemotherapy group.The clinical efficacy,pain degree[visual analogue scale(VAS)],frailty status[Fried frailty phenotype(FFP)],pulmonary function indexes[forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),FEV_(1)/FVC]and adverse reactions were compared between the two groups.Result The total effective rate in the particle implantation group was higher than that in the chemotherapy group,the difference was statistically significant(P<0.05).After treatment,the VAS and FFP scores in two groups were lower than those before treatment,the FVC,FEV_(1) and FEV_(1)/FVC were higher than those before treatment,and the VAS and FFP scores in particle implantation group were lower than those in chemotherapy group,the FVC,FEV_(1) and FEV_(1)/FVC were higher than those in chemotherapy group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion ^(125)I radioactive particle implantation in the treatment of elderly patients with non-small cell lung cancer can reverse frailty,inhibit cancer pain and improve lung function,and is relatively safe and effective.
