摘要
目的探究心源性休克患者血清解偶联蛋白2(UCP2)与血乳酸的相关性及两者对预后的预测价值。方法选取苏州大学附属常州老年病医院2017年3月至2022年4月收治的61例心源性休克患者作为研究对象,分别采集患者入院时动脉血及外周静脉血,检测血乳酸和UCP2水平,根据治疗28 d后预后生存情况将其分为死亡组和存活组。分析UCP2与血乳酸之间的相关性及两者对心源性休克预后的预测价值。以受试者操作特征(ROC)曲线评估血乳酸和UCP2对心源性休克患者预后的预测价值。结果死亡组患者UCP2、血乳酸、肌钙蛋白及胆红素水平均高于存活组,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,UCP2与血乳酸呈正相关(r=0.529,P<0.05)。Logistic回归分析结果显示,UCP2、血乳酸、肌钙蛋白及胆红素升高均为心源性休克预后的危险因素(P<0.05)。ROC曲线分析显示,UCP2、血乳酸及二者联合预测心源性休克预后的AUC分别为0.703、0.708、0.796。结论在心源性休克患者病情进展中,UCP2与血乳酸呈正相关,且UCP2、血乳酸对心源性休克预后具有较高的预测效能。
Objective To explore the correlation between serum uncoupling protein 2(UCP2)and blood lactic acid in patients with cardiogenic shock and their prognostic value.Methods A total of 61 patients with cardiogenic shock treated in Department of Critical Care Medicine from March 2017 to April 2022 were selected as research objects.Arterial blood and peripheral venous blood were collected to examine the levels of blood lactic acid and UCP2 at admission.The patients were divided into death group and survival group according to the survival after 28 days of treatment.The correlation between UCP2 and blood lactate acid and the factors influencing the prognosis of cardiogenic shock were analyzed.The predictive value of blood lactate acid and UCP2 for the prognosis of cardiogenic shock was evaluated using receiver operating characteristic(ROC)curve.Results The levels of UCP2,blood lactic acid,troponin and bilirubin in the death group were significantly higher than those in the survival group(all P<0.05).Pearson analysis showed that UCP2 was positively correlated with blood lactate acid(r=0.529,P<0.05).Logistic analysis showed that the increase of UCP2,blood lactate,troponin and bilirubin were all risk factors for the prognosis of cardiogenic shock(all P<0.05).ROC curve showed that the AUC values of UCP2,blood lactate acid and their combination in predicting the prognosis of cardiogenic shock were 0.703,0.708 and 0.796,respectively.Conclusion UCP2 is positively correlated with blood lactate acid inthe progression of cardiogenic shock.UCP2 and blood lactate acid have high predictive value for the prognosis of cardiogenic shock.
