纤维支气管镜灌洗结合抗感染治疗对重症肺部感染患者肺功能及预后的影响

Effect of Fiberoptic Bronchoscopy Lavage Combined with Anti-infective Therapy on Pulmonary Function and Prognosis in Patients with Severe Pulmonary Infection

目的探究纤维支气管镜灌洗结合抗感染治疗对重症肺部感染患者肺功能及预后的影响。方法选取2021年6月-2023年5月我院接收的重症肺部感染患者62例作为研究对象,根据随机数字表法分为对照组、研究组,各31例。对照组给予抗感染治疗,研究组给予纤维支气管镜灌洗结合抗感染治疗。比较两组临床疗效、恢复情况(咳痰消除时间、咳嗽消除时间以及体温恢复至正常时间、白细胞计数恢复至正常时间、住院时间)、血气指标水平(PaO_(2)、PaCO_(2)、SaO_(2))、炎症水平(PCT、CRP)、肺功能(MMV、FVC、FEV_(1)、FEV_(1)/FVC)。结果研究组治疗总有效率高于对照组(P<0.05);研究组咳痰消除时间、咳嗽消除时间以及体温恢复至正常时间、白细胞计数恢复至正常时间、住院时间均短于对照组(P<0.05);研究组治疗后PaO_(2)、SaO_(2)水平高于对照组,PaCO_(2)水平低于对照组(P<0.05);研究组治疗后PCT、CRP水平低于对照组(P<0.05);研究组治疗后MMV、FVC、FEV_(1)、FEV_(1)/FVC高于对照组(P<0.05)。结论纤维支气管镜灌洗结合抗感染治疗重症肺部感染患者的效果确切,可缩短患者症状改善时间,改善患者血气指标以及肺功能,减轻患者炎症反应,值得临床应用。

Objective To explore the effect of fiberoptic bronchoscopy lavage combined with anti-infective therapy on pulmonary function and prognosis in patients with severe pulmonary infection.Methods A total of 62 patients with severe pulmonary infection received in our hospital from June 2021 to May 2023 were selected as the research objects.According to the random number table method,they were divided into control group and study group,with 31 patients in each group.The control group was given anti-infection treatment,and the study group was given fiberoptic bronchoscopy lavage combined with anti-infection treatment.The clinical efficacy,recovery(expectoration elimination time,cough elimination time,body temperature recovery to normal time,white blood cell count recovery to normal time,hospitalization time),blood gas index level(PaO_(2),PaCO_(2),SaO_(2)),inflammation level(PCT,CRP),lung function(MMV,FVC,FEV_(1),FEV_(1)/FVC)were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).The sputum elimination time,cough elimination time,body temperature recovery to normal time,white blood cell count recovery to normal time and hospitalization time in the study group were shorter than those in the control group(P<0.05).After treatment,the levels of PaO_(2) and SaO_(2) in the study group were higher than those in the control group,and the level of PaCO_(2) was lower than that in the control group(P<0.05).After treatment,the levels of PCT and CRP in the study group were lower than those in the control group(P<0.05).After treatment,MMV,FVC,FEV_(1) and FEV_(1)/FVC in the study group were higher than those in the control group(P<0.05).Conclusion Fiberoptic bronchoscopy lavage combined with anti-infection is effective in the treatment of patients with severe pulmonary infection,which can shorten the time of symptom improvement,improve the level of blood gas index and lung function,and reduce the inflammatory response of patients.It is worthy of clinical application.
