上颌前牙修复中二氧化锆全瓷冠的应用及其对牙周组织、口腔内环境的影响

Application of Zirconia All Ceramic Crown in Maxillary Anterior Teeth Restoration and its Impact on Periodontal Tissue and Oral Environment

摘要 目的:研讨上颌前牙修复中二氧化锆(ZrO_(2))全瓷冠的应用及其对牙周组织健康、口腔内环境的影响。方法:选取2021年5月—2023年1月在许昌市中心医院行上颌前牙修复的84例患者作为研究对象。按照随机数表法将其分为两组,每组各42例。对照组行镍铬合金烤瓷冠修复,观察组行ZrO_(2)全瓷冠修复。比较两组患者的修复效果、修复前后牙周组织健康状况[菌斑指数(PLI)、牙龈指数(GI)、出血指数(BI)]、咀嚼功能以及龈下菌斑检测情况。结果:观察组患者修复后边缘密合度、修复体颜色及继发龋问题评级均高于对照组,差异均有统计学意义(χ^(2)=6.930、6.473、6.486,P<0.05)。修复后,观察组患者GI值、BI值及PLI值均低于对照组,差异均有统计学意义(t=3.335、2.954、2.792,P<0.05)。修复后,观察组患者咬合力、咀嚼效率均高于对照组,差异均有统计学意义(t=10.445、3.981,P<0.05)。观察组患者修复后龈下菌斑中牙龈卟啉单胞菌、具核梭形杆菌、福赛坦菌及伴放线放线杆菌检出率均低于对照组,差异均有统计学意义(χ^(2)=5.973、6.222、4.086、4.480,P<0.05)。结论:对上颌前牙缺损患者行Zro2全瓷冠修复效果显著,并可明显改善患者的咀嚼功能及口腔内环境,维持其牙周组织健康。 Objective:To investigate the application of zirconia(Zro_(2))all ceramic crowns in the restoration of maxillary anterior teeth and its impact on periodontal tissue health and oral environment.Methods:84 patients who were admitted to the hospital for maxillary anterior tooth restoration from May 2021 to January 2023 were selected as the study subjects.They were decided into the control group(42 cases,with nickel chromium alloy porcelain crown restoration)and the observation group(42 cases,with Zro_(2) all ceramic crown restoration)according to the random number table method.The restoration effect,periodontal tissue health status before and after restoration[Plaque Index(PLI),Gingival Index(GI),Bleeding Index(BI)],chewing function,and subgingival plaque detection were compared between the two groups.Results:The observation group showed higher levels of edge fit,estoration color,and secondary caries issues after repair compared to the control group(χ^(2)=6.930,6.473,6.486;P<0.05).After repair,the GI value,BI value,and PLI value of the observation group were lower than those of the control group(t=3.335,2.954,2.792;P<0.05).After repair,chewing function indicators(bite force and chewing efficiency)in the observation group were higher than those in the control group(t=10.445,3.981;P<0.05).The detection rates of Porphyromonas gingivalis,Fusobacterium nucleatum,Fusetan and Actinobacillus actinomycetes in the subgingival plaque in the observation group were lower than those in the control group after repair(χ^(2)=5.973,6.222,4.086,4.480;P<0.05).Conclusion:Zro_(2) all ceramic crowns had significant effect on patients with maxillary anterior tooth defects,and could significantly improve their chewing function and oral environment,maintaining periodontal tissue health.
